The Personal Support Worker provides personalized care and support to clients in their homes, promoting independence and well-being within the framework of VON Canada's policies and procedures.
Key Responsibilities- Assess and evaluate client needs to develop and implement personalized care plans.
- Provide personal care services, including bathing, dressing, and grooming, as well as light housekeeping and meal preparation.
- Support clients with daily activities, such as walking, climbing stairs, and managing medications.
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
- Collaborate with the healthcare team to identify and address client needs, and make recommendations for care plan adjustments.
- Document client progress and care plan updates in a timely and accurate manner.
- High school diploma or equivalent.
- CCA Certification in Nova Scotia or equivalent.
- Personal Support Worker (PSW) certification or equivalent.
- Standard First Aid Certificate or CPR level C Certificate.
- Experience working with elderly or ill individuals or children.
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics.
