Job Title: Home Attendant/Community Health Worker About the Role: We are seeking a skilled and compassionate Home Attendant/Community Health Worker to join our team at Chilliwack Home Health in Chilliwack, BC. As a key member of our multidisciplinary health care team, you will play a vital role in supporting client independence and promoting a safe work environment. Job Summary: * Provide assistance with various housekeeping duties and activities outlined in the care plan * Travel is a requirement of this position, and transportation arrangements must meet the operational requirements of FH * Use a personal vehicle and hold a valid BC Driver's License Responsibilities: * Observe clients and their environments, and report behavioural, physical, and/or cognitive changes as well as unsafe conditions to supervisor * Participate in staff meetings, quality improvement initiatives, committees, and in-services * Complete and maintain related records and documentation, such as communication books and progress reports * Perform housekeeping duties, such as sweeping and mopping floors, vacuuming, dusting, washing dishes, laundry, bed linen changes, and emptying garbage * Plan, prepare, and serve meals; provide assistance with clean up after meals * Assist with ordering and receiving groceries * Support set up and clean up pre and post personal care performed by Community Health Worker 2 or caregiver * Promote a safe and healthy working environment by observing routine precautions and infection control procedures * Demonstrate methods and provide basic information to clients in relation to housekeeping, grocery shopping, meal planning and preparation, leisure activities, etc. * Provide short-term respite in the home, by performing duties such as sitting, socializing, and engaging with clients in therapeutic activities * Provide companionship by performing home-based leisure activities with the client, such as engaging in conversation, reading with/or to the client, playing board games and/or card games Qualifications: * Grade 12 * Class V BC Driver's License and access to personal vehicle with appropriate business insurance coverage * FoodSafe * Recent, related experience of three months * Or an equivalent combination of education, training, and experience * Or other Qualifications determined to be reasonable and relevant to the level of work Skills and Abilities: * Home management skills * Physical ability to carry out the duties of the position * Ability to work independently and in cooperation with others * Ability to operate related equipment and technology (iPhones, computers) * Ability to communicate effectively, both verbally and in writing * Ability to organize and prioritize * Ability to observe and recognize changes in clients About Fraser Health: Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people.
