Chronic Disease Nurse, Diabetes Specialist

Fraser Health is seeking a skilled Chronic Disease Nurse to join our Diabetes Clinic team in Abbotsford, BC. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients with diabetes.

Responsibilities
- Develop and implement individualized care plans for patients with diabetes, in collaboration with other healthcare professionals.
- Provide education and support to patients and their families to promote self-management of diabetes and related co-morbidities.
- Conduct outpatient assessments and share information with other healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures related to diabetes care.
- Collaborate with other healthcare professionals to provide comprehensive care to patients with diabetes.
Education and Experience
Graduation from an approved school of Nursing. Two (2) years' recent related clinical experience assessing and treating patients with diabetes within a chronic disease management framework, including supporting self-management and accreditation as a certified Diabetes Educator (CDE), or an equivalent combination of education, training, and experience. Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Current CPR certification.
