About the Role

We are seeking a highly skilled Registered Nurse to join our Diabetes Education Program as a Chronic Disease Management Specialist. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients with diabetes.

Key Responsibilities

  • Assess and develop individualized care plans for patients with diabetes, in collaboration with other healthcare professionals.
  • Provide education and support to patients and their families on diabetes self-management, including medication management and lifestyle adjustments.
  • Conduct outpatient assessments and share information with other healthcare team members to ensure seamless care.
  • Participate in quality improvement initiatives and contribute to the development of clinical resources and education materials.
  • Maintain accurate and detailed records of patient interactions and care plans.

Requirements

  • Graduation from an approved school of nursing with a minimum of two years of recent related clinical experience in chronic disease management.
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Certification as a Certified Diabetes Educator (CDE) is an asset.
