  • Responsible for providing program management and leadership to the IPAC program for HMLTCH to ensure regulatory compliance and implementation of clinical and nonclinical care standards and best practices in accordance with the Fixing Long-Term Care Act, 2021 and ON Regulation 246/22.a.
  • Responsible for providing final oversight and submission support of RAI/MDS reporting and continued support of the RAI coordinator and assessment completion in the home;
  • Provides both clinical and administrative support to the nursing team and is responsible for coordinating risk management by fostering a culture of continuous quality improvement with a focus and emphasis on residents, and safety.
  • Accountable for assisting with the management of quality nursing care and services within the home.
  • Participates in the interdisciplinary team and works cooperatively with team members to ensure the coordination of care and treatment for the residents.
  • Ensures that nursing personnel provide treatments, medications, diets, rehabilitative nursing care, activities of daily living, protective service, and training in selfcare as stated in the care plan to ascertain that optimum comfort and care is provided;
  • Continuously improving nursing policies, procedures, and programs to ensure compliance with best practice research, and applicable legislative changes/requirements.
  • Works collaboratively with the Director of Care on special projects, reviews, investigations, policy and procedure development and implementation, and performs other duties as assigned;
  • Oversees the Pharmacy activities within the home and participate and support the resident admission, transfer and discharge process; working closely with social workers and registered staff to ensure smooth transition and the development of supportive care plans.;
  • Assumes responsibility for the nursing department in the Director of Care's absence.
  • Conjointly with the Director of Care, is responsible for daily operations of clinical services including: monitoring of required clinical programs including Skin and Wound, Continence Care, Pain Management, Fall Prevention, RAI MDS, Nursing Rehab & Restorative Care.
  • Provides leadership and direction to staff involved in the provision of continuing care by fostering and promoting new initiatives; interpreting and implementing care related policies and procedures.
  • Ensures compliance with all relevant legislation, Humber Meadows policies/procedures, and accreditation requirements;
  • Maintains an active presence in the Resident Home Areas mentoring staff while overseeing the provision of quality resident care, while supporting RHA Manager inquiries;
  • Identifies areas of improvement for staff development and works with the Director of Care to ensure overall effectiveness of training by acting as the program lead for orientation, inservice education, and onthejob training as required;
  • Directly involved in the recruitment, disciplining and termination, performance management of IPAC, RAI and Admissions team members.
  • The effective leadership and management of the nursing service in HMLTC to ensure the efficient utilization of human, physical, and financial resources to meet or exceed Ministry care standards.
  • Maintain and update audit, compliance and legal documentation, including analyzing patient and staff data
  • Ensure equipment and supplies are used properly and effectively manages utilization of nursing supplies;
  • In collaboration with senior operations team assists in the development of the mission, vision, value statement for the corporation and Home;
  • Works with the Directors of Care on strategic planning and the setting of annual goals and objectives;
  • Coordinates the ordering, inventory management, distribution, and appropriate and safe use of supplies and equipment for the nursing department;
  • Supports in the development of individualized health care plans, computerized care plans and charts of the residents as required by the Charitable Institutions Act and Regulation 69, in keeping with the Fixing Long Term Care Act and Regulation, the professional licensing standards and in accordance with the standards and guidelines of the Ministry of Health as provided in the Long Term Care Homes Act and its Regulations;
  • Other duties as assigned.
