A day in the role of a Restorative Care Aide at Hawthorne Place Care Centre.


As a Restorative Care Aide at Hawthorne Place Care Centre, you will be given high expectations, as well as an expectation to continue personal growth and professional growth.

In this position, you will be gifted with a rewarding experience every day in the form of being able to see the exact impact you place on our residents.

As a Restorative Care Aide, you will report directly to the Programs Manager.

This position is responsible for but not limited to:

  • ROM, exercise and equipment needs (i.e.
ambulatory and assistive devices) ensures accurate records as required including:

develop policies and procedures for rehab programs, participation tracking, monthly statistics, Intake documentation, Quarterly documentation, MDS/RAPS, Care plan updates- Restorative Care under the direction of the Physiotherapy and/or Occupational Therapist.- Provide programs that meet the physical needs of the residents including: walking, ROM exercises, large group exercises, etc.)- Process and monitor all referrals and modify programs when necessary to ensure individual needs are met- Audit and evaluate restorative care program to residents assessed as requiring this program- Evaluate the effectiveness of therapy(ies) provided to resident and revise the plan of care according- Assist with the evaluation of therapies provided in collaboration with Physiotherapist and/or Occupational Therapist.- Has good understanding of the Nursing Restorative Philosophy- Work closely with Unit Supervisor and interdisciplinary team to establish nursing restorative services for residents in consultation interdisciplinary team updates care plans, evaluates and documents accordingly- Participate in department meetings, unit meetings, care plan rounds, and interdisciplinary care conferences and pre-admission care conferences.- Ensure effective communication and good human relations with residents, residents' families, volunteers, and staff of other departments.- Required to understand the nature and meaning of quality indicators used by the Home- Participate and conduct audits according to the schedule in the Quality Improvement Program.

Qualifications:

The qualifications needed to join our family at Hawthorne Place Care Centre are as follows:

  • Has successfully completed a training programs in restorative care or is enrolled in such a programs, or has successfully completed relevant training course provided at the Home by a qualified Therapist (Occupational Therapist or Physiotherapist)
  • Experience working in a long term care setting
  • Committed to promoting residents' well being
  • Ability to develop, implement and evaluate programs
  • Demonstrated leadership, communication and interpersonal skills
  • Computer literate
  • CPR and First Aid qualifications
  • Knowledge of RAI-MDS
  • AIS certified, if not must be completed successfully within one (1) year of being hired.
  • Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory references.
