about the role:

The Health Promoter works to enhance the health and well-being of 2SLGBTQ communities, specifically 2SLGBTQ youth and adults. This role encompasses a broad range of responsibilities aimed at addressing social determinants of health and fostering a supportive environment that prioritizes the mental, physical, and emotional well-being of 2SLGBTQ community members.

Scope of the Mandate:

The Health Promoter plans and implements groups to enhance the health and well-being of 2SLGBTQ communities, supports outreach activities, and conducts intake to attract suitable service users for programs. The position is responsible for developing and delivering educational programs and workshops on health issues affecting 2SLGTBTQ communities.

Additionally, the health promoter creates and disseminates information and educational materials using evidence-informed communications strategies, engages in community development activities, and uses research evidence and community feedback to guide the development of programs, resources, and policy briefs.

day to day responsibilities:

Health Communications

  • Creates communications materials such as newsletters, brochures, posters, and fact sheets, working with writers, graphic artists, and others as needed.
  • Collaborates with the communications team to promote programs to a targeted audience.
  • Oversees the maintenance and functionality of the website and other electronic media used by the CPCBI programs.
  • Responds to program inquiries and provides appropriate information or links to other resources.

Program and Community Development

  • Identifies the ongoing health and social issues of 2SLGBTQ communities through individual contacts, surveys, literature searches, and work with community organizations.
  • Designs and implements health promotion programs tailored to the unique needs of 2SLGBTQ individuals, including those with intersectional identities.
  • Monitors and evaluates the effectiveness of health promotion programs and initiatives, making adjustment as needed.
  • Plans and facilitates support groups and community meetings to understand and address health concerns within the 2SLGBTQ community.
  • Provides expert information and referrals to 2SLGBTQ community members, service providers, and other professionals.
  • Facilitates and/or administers ongoing group programs offered by the 2SLGBTQ community programs, responding to the ongoing and evolving needs of participants.
  • Develops and implements evaluation frameworks to assess the impact and effectiveness of health promotion programs and initiatives.
  • Provides training and consultation to community members, service providers, students, and others on the needs of 2SLGBTQ parents and families.

Outreach, Engagement, and Partnership Development

  • Coordinates all aspects of health promotion groups, including outreach, registration and health promotion activities.
  • Identifies and leverages existing community resources to support the success and impact of health promotion programming.
  • Implements evidence-informed, community-specific outreach strategies.
  • Evaluates the success of outreach initiatives and makes improvements to ensure that outreach has a high impact.
  • Supports the development of partnerships with local community organizations, local health departments, schools, businesses, etc.
  • Facilitates participatory approaches to engage community members in the development and implementation of health promotion activities.
  • Collaborates with partner organizations to share relevant information and resources, ensuring consistent communication and capacity-building.

Administration

  • Works with colleagues and the program management team to identify and prioritize issues, plan overall program goals and objectives and evaluate their effectiveness.
  • Collaborates with team members to plan and implement health promotion initiatives and interprets, implements, and supports established policies and procedures.
  • Makes recommendations and contributes to policy formulation and quality improvement initiatives.
  • Participates in the development, implementation and evaluation of programs and standards of practice for service delivery and accreditation.
  • Maintains accurate records of program attendance, participant feedback, and outcomes, and prepares reports as required.
  • Attends and participates in Sherbourne meetings as assigned by management.

To thrive in this role, you'll need

education/ experience/ knowledge and skills:

  • Education in health promotion or health/social sciences at a Master's level or equivalent formal training
  • Demonstrated knowledge of 2SLGBTQ youth, their families and broader social environment for diverse 2SLGBTQ communities
  • Excellent written and oral communication skills including writing for a variety of print and social media.
  • Significant experience in group facilitation, community education and training.
  • Good administrative and organizational skills including knowledge of computer software, project and event management and the production of narrative and statistical reports.
