Our SILP program is looking for an enthusiastic Human Services Counselor.

- **Duties and Support given to the members**:
  • Teach, coach, assist and support independent living skills.
  • Setting goals and the path to achieve them.
  • Cooking skills and healthy food choices.
  • Maintaining good personal hygiene, including laundry.
  • Maintaining good apartment hygiene.
  • Monitoring medication and reminders.
  • Budgeting and banking.
  • Personal shopping (groceries, clothing, sundries, family gifts, etc.).
  • Attending (as required) appointments with medical professionals, follow up as necessary.
  • Attaining employment as necessary.
  • Finding living arrangements as required.
  • Utilizing community resources (transportation, activities, medical, etc.).
  • Provide advice/emotional support as needed.
  • Assist in establishing and maintaining good support connections with family/friends.
  • Documentation
  • Document data from each visit on the visit report including any missed/refused visits.
  • Update health logs after each medical appointment.
  • Update financial records.
  • Ensure relationship agreement, visit reports get updated whenever there is a change in service.
  • Report concern of safety, and incidents.
  • Follow a visit schedule according to the visit reports and caseload hours.
  • Follow company policies.
  • Communicate changes in support needs to the supervisor.
  • Work with others and build healthy relationships.

Required skills and qualifications

  • Preference will be made for a Human services diploma or equivalent.
  • First-Aid and CPR certificate.
  • Clear SD check and criminal record check (including vulnerable sector check).
  • Valid driver's license to transport members you support, with $1,000,000 insurance liability.
  • Spoken and written English (writing reports in English).
  • Awareness of financial management.
  • Excellent organizational skills.
  • Ability to work independently with excellent problemsolving skills.
  • Ability to work evenings, and weekend rotation.
  • Ability to attend work consistently.
  • Effective communication.
  • Excellent people skills.
  • Crisis management skills.
