Service Navigator 2, Complex Client Care (Slaight Pathway)

ABOUT THE ROLE

The Service Navigator 2, Complex Client Care (Slaight Pathway) role is a full-time permanent position responsible for service planning, navigation, and brief supportive counseling for children, youth, and families dealing with mental health issues. The primary focus is on facilitating transitions from SickKids Emergency Department to community-based mental health services. Collaboration with partners at SickKids and the Garry Hurvitz Centre for Community Mental Health is essential to support this clinical transition pathway.

Salary range: $63,071.00 to $73,447.00 per annum.

WHAT YOU WILL DO


- Review inbound referrals from SickKids Emergency Department to Help Ahead.


- Collaborate with GH-CCMH case navigator to determine the transition pathway.


- Connect with children, youth, and families to provide necessary support and service planning.


- Educate families on mental health and available services.


- Facilitate service planning and maintain relationships with stakeholders.

THE 'MUST HAVES'

Requirements for this role include a Bachelor's or Master's Degree in Social Work, Psychology, or Health-Related field, along with relevant experience and professional certifications. Strong clinical assessment, treatment planning skills, and knowledge of mental health disorders are essential.

THE 'NICE TO HAVES'

Preferred qualifications include familiarity with department goals, ability to work with cross-sectoral teams, and extensive knowledge of Child and Youth Mental Health services. Customer service orientation, cultural sensitivity, and fluency in multiple languages are beneficial.
