Transitional Care Unit

It is a condition of this posting that your home unit canaccommodate your temporary absence in the event

The kinesiologist has a direct reporting relationship to theManager of the Transitional Care Unit‐(TCU) with a matrix

reporting relationship for kinesiology practice to the Director,Health Professions and Collaborative Practice. The

kinesiologist will work in partnership with the interprofessionalteam to develop the plan of care. The kinesiologist is

responsible for conducting initial and ongoing physical activityassessments as well as organizing, planning,

coordinating, delivering and supervising care plans. Thekinesiologist will also provide individual and group education

for the patient.

As a member of the interprofessional team the kinesiologist willactively participate in team rounds.

The TCU kinesiologist will perform all duties of a kinesiologist,following the scope of practice defined by the College

of Kinesiologists of Ontario.

EMPLOYMENT REQUIREMENTS:

Education:

Bachelor of Science degree in Kinesiology

Certifications:

Current registration in good standing with the College ofKinesiologists of Ontario required

BCLS

CSEP‐ Preferred. Professional Fitness and LifestyleConsultant

Member in good standing with Ontario Kinesiology Association

Required Skills:

Demonstrate critical thinking, problem solving and conflictresolution skills

Experience in the care of geriatric patients

Minimum 3 year experience in a hospital or long term caresetting

Experience working in an interprofessional team

Good verbal and written communication skills

Computer skills including use of power point excel and word

Preferred skills:

Competency and or experience in program administration

Specific additional training in exercise physiology,participant counseling or other related health care activities.

Experience using an electronic medical record

Salary Range: $39.160 to 50.770
