RESPONSIBILITIES


The Community Social Supports Coordinator is the primary point of contact to meet and support clients who access support services.

The Community Social Supports Coordinator will support clients with understanding of resources available and connecting clients with the appropriate support agency or organization.

The Community Social Supports Coordinator assists the Executive Director with the overall direction and outcomes of the CDFCSSA programs, especially with respect to direct service delivery and including:

Leadership and Supervision

  • Supports and fosters the mission, goals, and guiding principles of the CDFCSSA Program;
  • Assists the Executive Director in modeling and teaching social support practices to volunteers and participants during program implementation.
  • Supports and coaches' participants, providing appropriate resources to do the work, training and development as necessary.

Program Planning, Monitoring and Evaluation

  • Works with the Executive Director, other CDFCSSA staff and the CDFCSSA Board to plan/design, implement, monitor and evaluate programs for individuals;
  • Facilitate and conduct the intake of clients using trauma informed and socially focused interview skills;
  • Using appropriate assessment tools, compiles a social history of a client and identifies various needs and supports available
  • Supports clients by making connections or referral appointments as the client so desires.
  • Provides case management services provides supports to clients prior to support groups and continue to help support the client postsupport group as their needs shift or evolve
  • Maintain contact throughout the process and followup defining the impact and improvements while determining if there are other needs;
  • Works with the Executive Director and other CDFCSSA staff to ensure that program intent, delivery and standards are followed and that social needs are identified and addressed as needed.
  • Assists the Executive Director with efficient and timely monitoring, reporting, assessment and recommendations based on program statistics, outcomes and achievements.
  • Continue to review, update and, maintain and update a relevant asset map;

Operational and Financial Management

  • Assists the Executive Director with financial planning and administration of CDFCSSA programs;
  • Assists the Executive Director with daytoday operations by maintaining adequate, appropriate operational and statistical records of all applicable program activities and assisting with external and internal communications;

Client and Public Relations

  • Promotes and practices excellent public relations with clients, program participants, their families, other CDFCSSA clients and personnel, and the community in general;
  • Develop a rapport with clients and complete a comprehensive social health history to determine needs;
  • Facilitate a relationship with the client that is inclusive, and trauma informed, provide a safe environment that helps bolster empowerment of the client.
  • Recognize the individual needs and learning style of each client. Provide appropriate coaching of clients, helping them to set realistic goals and timelines;
  • Assists the Executive Director with timely and effective problem solving, grievance handling and dispute resolution;
  • Assists with general CDFCSSA service to the public as appropriate, necessary, and feasible.

Community Partnership and Liaisons

  • As directed by the Executive Director or initiated by staff, cooperates with other community programs serving CDFCSSA participants, resulting in linked services where applicable and feasible;
  • As directed by the Executive Director, develops and maintains effective communication links with other related agencies, funding bodies and government departments.
  • Be an ambassador within the community of social services, liaison with agencies as needed about client and potential needs and work collaboratively with support agencies or organizations;

Board Development/ Management

  • Assists the Executive Director in ensuring that Board members have the information and resources they need to work effectively.

GENERAL COMPETENCIES

  • Is familiar with CDFCSSA policies and practice and maintains such familiarity in terms of policy revisions and changes;
  • Has full knowledge of and commitment to the CDFCSSA Code of Ethics, mission, goals and guiding principles as outlined in the policy;
  • Has effective interpersonal planning and organizational skills, exhibiting a generally positive approach and a high degree of personal initiative;
  • Is able to work cooperatively in a team, facilitating a team environment through personal behavior, work contributions and the sharing of knowledge.
