BC Emergency Health Services (BCEHS) is hiring a Patient Safety Investigator.

Role Summary

In accordance with the Mission, Vision and Values, and strategic directions of PHSA safety, including both patient and employee safety, is a priority and a responsibility shared by everyone at PHSA's Agencies/Divisions/Services, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Manager, Indigenous Health, the Patient Safety Investigator is responsible for patient safety event review processes. In collaboration with other members of the Quality, Safety, Risk Management & Accreditation team and operational staff, the Patient Safety Investigator promotes a culture of safety across the organization by providing leadership for reviews following serious patient safety events.

The goal is the reduction of preventable patient injuries and deaths occurring during the provision of healthcare services. An important focus will be on learning from clinical practice, specifically healthcare system failures, through the comprehensive retrospective analysis of critical incidents occurring throughout the region. The position also involves active participation and leadership in the promotion of a culture of safety.

The Patient Safety Investigator will also become involved in various projects to promote quality and safety improvements in clinical practice across the Health Authority specifically those arising from patient safety event reviews under their leadership.

What you'll do

Working collaboratively with the other members of the existing site Quality, Safety, Risk Management & Accreditation team, develops and maintains excellence in the methodology of retrospective analysis of critical incidents (CIs). Leads and/or participates in Critical Incident Review Committees following a critical incident. As required, acts as a mentor to Critical Incident Review Committees (whether individual or team) who are performing reviews. Ensures local Quality, Safety, Risk Management & Accreditation staff receives ongoing communication about Critical Incident Reviews in their geographic area by participation at local team meetings and other means. Leads the process of trending both incoming and completed site reviews and ensures the spread of relevant information to all levels within the organization. Assists in the development of process to ensure involvement of patients and family members in the review process Provides support and assistance to operational staff throughout the disclosure process. Participates in the ongoing continuing education and training of staff involved in Critical Incident Reviews following serious safety events Develops and maintains expertise in the methodology of prospective analysis of healthcare system processes such as Failure Mode Effects Analysis (FMEA) and participates in the performance of FMEAs as required Actively participates in other regional educational and culture change activities within the Quality, Safety, Risk Management & Accreditation portfolio.

What you bring

Qualifications

A level of education, training, and experience equivalent to an undergraduate degree in social sciences, health, or related profession. Professional clinical background and/or conflict resolution expertise preferred. Minimum three to five years' experience working in a large, multi-faceted health or social/human service organization required.

Skills & Knowledge

Must have excellent analytical skills, problem solving abilities, communication skills, judgment and interpersonal skills. Integrity, dependability, honesty, initiative, tact and high ethical standards are required. Experience in handling sensitive patient/family interactions essential. Must have basic computer navigation skills. Lived experience working directly with or alongside Indigenous communities and/ or Indigenous health organizations Investigation skills will be an asset. Travel may be required as needed.

Indigenous Cultural Safety and Diversity

Knowledge of Canadian colonial impacts on Indigenous people in social and health contexts, supported by significant knowledge of Canadian and Indigenous ideologies, including knowledge, and understanding of Indigenous cultural safety, cultural humility, Indigenous-specific racism and anti-racism. Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action (2015), In Plain Site Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and Reclaiming Power and Place Missing and Murdered Indigenous Women & Girls calls for justice, and how they intersect across the health care system. Supports team members on their learning journey, ensuring education strategy for team/department to implement Indigenous Cultural Safety at a practical level. Works collaboratively with appropriate Indigenous teams/departments to ensure ICS lens applied holistically. Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC and BC's Declaration on the Rights of Indigenous Peoples Act Knowledge of social, economic, political and historical realities impacting indigenous communities and familiarity with Indigenous Cultural Safety and anti-racism and accompanying reports (BC DRIPA, TRC, etc.)

While BCEHS would consider a foundation in Indigenous cultural safety to be a strong asset for consideration, our commitment to enhancing Indigenous cultural safety includes providing all employees with access to resources and training programs in partnership with San'yas Core Health Indigenous Cultural Safety Training and Anti-Indigenous Racism Response Training.
