Purpose of the position:


Pivotal to the success of the pharmacy, the role is split between working at Church Street and working with our ACE team.

As a member of the Adherence and Community Engagement (ACE) Team, you'll play a pivotal role in revolutionizing healthcare delivery for residents of Edmonton's inner city.

In this predominantly clinical position, you'll lead a multidisciplinary team in providing daily care to marginalized individuals facing homelessness and addiction challenges.

Your responsibilities will include comprehensive case management for 40-45 patients, collaborating closely with outpatient and community-based healthcare teams to enhance patients' health and social well-being.

Additionally, you'll have the opportunity to engage in research activities and contribute to provincial strategies and action plans for managing sexually transmitted and blood-borne infections (STBBI) and advancing harm reduction initiatives in Alberta.

This full-time role entails weekday shifts, along with short and efficient 5-6 hour rotations on Saturdays and Sundays, as per the schedule.

As a Pharmacist at Church Street, you will provide guidance on the safe, effective, and appropriate use of medications to achieve optimal therapy outcomes.

Your role involves delivering pharmaceutical care, leveraging clinical knowledge, skills, and judgment to assess and address patients' drug-related needs. Ensuring compliance with pharmacy practice regulations, Standards of Practice, and organizational Code of Ethics is a key responsibility.

We are particularly interested in candidates with a strong passion for addictions, mental health, and infectious diseases such as hepatitis C and HIV.

Critical thinking skills are essential, and you should thrive in a challenging and dynamic environment. Adaptability, forward-thinking, and creative problem-solving abilities are highly valued traits in this role. Since ACE operates as a teaching team, willingness to supervise and mentor students is essential.

Mint Health + Drugs:

Church Street pharmacists serve a diverse and high-need population, including vulnerable patients, individuals with substance use disorders, LGBTQ individuals, and those with mental health concerns.


Responsibilities:

  • Accurately interpret and fill prescription orders, ensuring the correct medication, dosage, and instructions are provided to patients.
  • Provide patients with thorough and precise medication counselling, addressing potential side effects, interactions, and proper usage.
  • Collaborate with healthcare providers to review and optimize medication therapy for patients.
  • Monitor for drug interactions, contraindications, and potential adverse effects.
  • Ensure compliance with all federal and provincial pharmacy laws and regulations.
  • Maintain accurate and complete patient records and medication profiles.
  • Interact with patients courteously and professionally, addressing their needs and concerns.
  • Manage pharmacy inventory, including ordering, restocking, and monitoring expiration dates.
  • Develop and implement plans to monitor the patient's progress, reinforce and facilitate medication adherence, and assess therapeutic outcomes
  • Document actions taken in the care of the patient and communicate with other health care providers where appropriate
  • Refer the patient and the patient's caregiver to relevant agencies where appropriate
  • Develop and promote healthy living on an individual or community basis through the promotion of positive choices that enhance both physical and mental health
  • Identify behavior associated with increased health risks and take an active role in educating those at risk
  • Screen patients at risk for chronic diseases or for diagnostic purposes for early identification and referral
  • Engagement in the collaborative care of patients; this may include preparing status reports, attending meetings and/or rounds with various care teams involved in the patient's health management
  • Commitment to improving patient's quality of life and social determinants of health
  • Learning and applying sound knowledge of local community services/resources that the patient can access for support or basic needs
  • Treating patients in a non-judgemental way, including having a knowledge of and being a strong advocate
  • Clinical assessment skills and understanding of therapies used in the treatment of opioid dependence for harm reduction strategies
Education and Experience

  • Bachelor's/Doctorate of Pharmacy
  • A member in good standing with the Alberta College of Pharmacy, including a valid license
  • Minimum two to three years of experience within a retail pharmacy environment
  • APA designation required or working towards
  • Injection certification required
Qualifications and Skills

  • Computer proficiency with Word, Excel spreadsheets, Kroll, Netcare, and other database programs.
  • Solid interpersonal, communication and customer service skills with both patients and other healthcare professionals
