Opportunity Overview:

The Clinical Pharmacist I plays a crucial role in ensuring the safe, effective, and efficient delivery of pharmaceutical services to patients within the ambulatory care framework, specifically focusing on the Home Parenteral Therapy (HPT) Program. This position involves the management of medication therapy for patients, where the pharmacist assesses for potential or actual drug-related issues both initially and continuously. Working collaboratively within a multidisciplinary team, the pharmacist intervenes to resolve or prevent drug-related complications, thereby enhancing patient outcomes.

Key Responsibilities:

As a Clinical Pharmacist I, your responsibilities will include:

  • Participating actively in daily program meetings to discuss patient care.
  • Collaborating with healthcare team members to develop comprehensive medication treatment plans.
  • Implementing and modifying medication therapy, which includes selecting, initiating, and administering medications.
  • Documenting both expected and actual outcomes, as well as clinical interventions made.
  • Providing drug information and medication counseling services to patients and their families.
  • Ensuring accurate and efficient dispensing of medications while fulfilling administrative duties.
  • Serving as a mentor and preceptor for pharmacy students and residents during their rotations.

Required Qualifications:

Candidates must possess a Bachelor of Science in Pharmacy and be actively registered or eligible for registration with the Alberta College of Pharmacy (ACP).

Additional Required Qualifications:

The following qualifications are also necessary:

  • Additional Prescribing Authorization.
  • Completion of an accredited hospital residency program or Pharm D program, or at least one year of recent experience in a large acute care hospital pharmacy setting.
  • Recent experience in a proactive clinical practice as part of a multidisciplinary healthcare team.
  • Fluency in English, both written and verbal.
  • Proficiency in drug information systems and word processing.
  • Ability to apply evidence-based medicine in direct patient care.
  • Commitment to ongoing self-directed learning and professional development.
  • Strong communication and interpersonal skills, with demonstrated organizational and problem-solving abilities.
  • Capacity to make independent decisions and manage time effectively, including prioritizing tasks and multitasking.
  • Recent experience in Home Parenteral Therapy within the last six months.

Preferred Qualifications:

Preferred qualifications include:

  • Active membership in the Canadian Society of Hospital Pharmacists (CSHP).
  • Current competency in sterile product preparation.
  • Familiarity with the Connect Care and Kroll computer systems.
  • Experience in Home Parenteral Therapy.
