Position Overview:
The Clinical Pharmacist I plays a vital role in ensuring the safe, effective, and efficient delivery of pharmaceutical care to patients within the ambulatory care environment, specifically focusing on the Home Parenteral Therapy (HPT) Program. This position is integral in managing medication therapies, where the pharmacist assesses patients for potential or existing drug-related issues, collaborating with a multidisciplinary team to intervene and enhance patient outcomes.
Key Responsibilities:
As a Clinical Pharmacist I, your primary responsibilities will include:
- Providing comprehensive medication management and therapy assessments for patients.
- Actively participating in daily team meetings to discuss patient care strategies.
- Establishing and maintaining professional relationships with patients and their families to develop tailored medication treatment plans.
- Implementing medication plans, which may involve selecting, initiating, modifying, and administering therapy.
- Documenting clinical interventions and monitoring both expected and actual patient outcomes.
- Delivering drug information and medication counseling services to ensure safe dispensing practices.
- Performing necessary administrative duties related to the HPT program.
- Mentoring pharmacy students and residents during their clinical rotations.
Required Qualifications:
Candidates must possess a Bachelor of Science in Pharmacy and be actively registered or eligible for registration with the Alberta College of Pharmacy (ACP).
Additional Required Qualifications:
The following qualifications are essential:
- Additional Prescribing Authorization.
- Completion of an accredited hospital residency program or Pharm D program, or at least one year of recent experience in a large acute care hospital pharmacy setting.
- Recent involvement in a proactive clinical practice as part of a multidisciplinary healthcare team.
- Fluency in English, both written and verbal.
- Proficiency in drug information systems and word processing.
- Ability to apply evidence-based medicine in patient care.
- Commitment to ongoing professional development and self-directed learning.
- Strong communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a team, with effective time management skills.
- Recent experience in HPT within the last six months.
Preferred Qualifications:
Preference will be given to candidates who demonstrate:
- Active membership in the Canadian Society of Hospital Pharmacists (CSHP).
- Current competency in sterile product preparation.
- Familiarity with the Connect Care and Kroll computer systems.
- Experience in Home Parenteral Therapy.
