Pharmacist I Job Description

We are seeking a highly skilled Pharmacist I to join our team at Alberta Health Services. As a key member of our interprofessional team, you will play a vital role in providing clinical services to home living clients who require medication support.

Key Responsibilities:
  • Provide clinical services to home living clients who may require various medication support needs.
  • Work collaboratively with an interprofessional team, including case managers, nurse practitioners, and physicians.
  • Intervene to resolve or prevent drug-related problems to optimize client health outcomes and support independent living.
  • Coordinate and integrate medication therapy management services into client care plans.
Job Requirements:
  • Bachelor of Science in Pharmacy.
  • Active or eligible for registration with the Alberta College of Pharmacy (ACP).
Preferred Qualifications:
  • Recent pharmacy experience providing care for geriatrics, complex care, persons with multiple co-morbidities, and multiple medications.
  • Community pharmacy experience.
