Position Information:

  • Interprofessional Spine Assessment and Education Clinic (ISAEC) is a program funded by the Province of Ontario's Ministry of Health and Long-Term Care. It is an innovative, upstream, sharedcare model of care in which eligible patients receive rapid low back pain assessment, education and evidencebased selfmanagement plans. It is designed to decrease the prevalence of unmanageable chronic low back pain, reduce unnecessary diagnostic imaging as well as unnecessary specialist referral.
  • The Ottawa Hospital is the physical hub for the Champlain region's ISAEC program although ISAEC utilizes a decentralized hub and spoke delivery of care model with both private and public health care involvement. This model allows for improved access to community based care as well as streamlined referral to specialists as indicated.
  • The Champlain LHIN has a dedicated Practice Leader (PL) as well as Advanced Practice Clinicians (APCs) in both Family Health Teams and independent practices in the community. The PL is located at the Ottawa Hospital, Civic campus. All regional ISAECs are connected through a centralized referral and clinic management system.
  • Temporary until approximately April 1, 2024
    Position Description:
  • As an integral member of the ISAEC team, the PL is an innovative, client service oriented clinical expert with advanced knowledge, skills and experience in low back pain treatment and management, with both clinical care and leadership responsibilities. The PL acts as the first and primary point of contact for consultation and assessment for ISAEC patients referred to the hospital site. This may include patients who are seeing an ISAEC practitioner for the first time, and those referred on by other ISAEC practitioners for secondary advanced assessment, The PL provides a defined program of care to the patient with an emphasis on education, self
- management and health promotion strategies; develops a collaborative care plan with the patient and their primary care provider and documents progress against the care plan; and identifies signs and symptoms requiring a change in the plan of care. The PL is trained to requisitions diagnostic imaging/testing if required, or referral to a physician specialist or for specific spinal procedures.
  • The PL acts as the program manager for the regional ISAEC program and has leadership responsibilities incorporating all aspects of regional day to day operations. This includes HR/people management for the regional ISAEC team of clinicians and administrative personnel, budget development and management, clinical leadership and mentoring including regular training for all ISAEC staff, liaison with local healthcare partners and reporting to business partners and stakeholders. It is also imperative that the PL applies learning and accountability consistent with a just culture, providing staff with written instruction on safety precautions where appropriate, ensuring that staff adhere to standards of safe work and patient care, and closing the loop on staff and patient safety issues to ensure continuous service improvement.

Rotation/Shifts:

  • Days, 8 hour shifts

Basic Requirements:

  • Four (4) year Undergraduate Degree
  • Entrytopractice program in Physical Therapy, or Chiropractic Medicine or Nurse Practitioner program;
  • Completion of a postgraduate musculoskeletalrelated credentialing program, or alternatively a total of 5 or more years of related clinical experience in the musculoskeletalspine area;
  • Member in good standing of respective professional/legislative college at the provincial level;
  • Member of related national/provincial professional association (e.g., OCA, OPA, NPAO);
  • A minimum of 5 years of clinical experience in the musculoskeletalspine area;
  • Excellent knowledge of principles of management and research based practice;
  • Excellent knowledge of spine related anatomy, pathology and physiology;
  • Excellent knowledge of spinal assessment techniques, and evidence based MSK treatment options for low back and chronic pain;
  • Knowledge of health care disciplines in Clinical Services and their role in patient care;
  • Knowledge and ability to enhance continuum of care;
  • Knowledge of statistics and ability to analyze fiscal and patient care data;
  • Knowledge of theoretical frameworks and research methods;
  • Excellent skills in oral and written communication;
  • Ability to interact with large groups;
  • Group facilitation and adult education skills including delivering complex or difficult information to patients;
  • Excellent interpersonal skills;
  • Ability to deal with difficult situations and crisis;
  • Ability to implement, facilitate, and lead change;
  • Proven ability in working effectively with all members of health care team;
  • Ability to respond to large numbers of simultaneous demands from patients, families, staff, physicians, Material Management, Human Resources, etc
