  • Performs comprehensive patient assessments to evaluate nutritional needs, deficits, and educational requirements for patients
  • Assists with the development of individualized patient nutritional plans, with direct participation from the patient and family
  • Provide dietary guidance and educational support for patients, promoting healthier lifestyle choices to help manage chronic conditions
  • Maintain accurate and detailed documentation, including assessments, treatment plans, progress notes and discharge summaries


Qualifications
  • Completion of a Bachelor's or Master's degree in an accredited Food & Nutrition program
  • Registration with the College of Dietitians of Ontario required
  • Member in good standing with the Dietitians of Canada preferred
  • Minimum 3-5 years experience working as a Registered Dietitian
  • Experience working in a community setting is an asset
  • Valid Ontario driver's license and a reliable vehicle
  • Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting)
