West Park Healthcare Centre helps patients get their lives back by providing specialized rehabilitative and complex care after a life‐altering illness or injury such as lung disease, amputation, stroke, and traumatic musculoskeletal injuries.

West Park is building a new hospital and transforming its 27-acre site into an integrated campus of care, enabling the hospital to evolve its rehabilitative programs to meet Ontario’s future healthcare needs.

**Qualifications**:

- B.Sc. in Nutrition or recognized equivalent, successful completion of an accredited dietetic internship
- Master’s preferred
- Registered with the College of Dieticians of Ontario
- Eligibility for membership for Dieticians of Canada
- 1-2 years’ experience
- Demonstrated excellence in clinical nutrition, clinical assessment and interventions
- Experience with g-tubes/j-tubes in patient care
- Interpersonal and communication skills (both written and verbal)
- Organizational skills
- Demonstrated ability to be self-directed, seeking direction when appropriate
- Ability to work effectively with all levels of staff and within a transdisciplinary team
- Ability to function within a consult model
- Minimum of one year recent clinical experience preferred
- Evidence of good performance history and attendance record
- Ability to work in a manner that exemplifies the core values of West Park: Excellence, Respect, Trust, Collaboration and Accountability
- Should be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors

**Accountabilities**:

- Provide nutritional assessment, make recommendations for the treatment and prevention of nutrition related disorders and coordinate nutritional care plan for patients and residents.
- Provide nutritional education to patients/residents, family members and allied health professionals and students.
- Evaluate effectiveness of nutrition plan of care.
- Participate in clinical evaluation related to clinical nutrition.
- Pursue continuous professional development.
- Complete timely and thorough clinical documentation & complete clinical workload.
- Ensure advances in the profession and best practice.
- Communicate with transdisciplinary team regarding nutrition care plan.
- Communicate with food service department regarding diet changes r/t patient/resident care.
