The Downtown East Toronto Ontario Health Team (DET OHT) is a group of 43 health and social service provider organizations (Team Members) who have deep roots in the community and is committed to creating an integrated system of care. The vision of the DET OHT is to provide our community with seamless access to inclusive, high quality, and responsive care through one connected system of diverse providers and community members who enthusiastically embody holistic approaches to improving population and individual health and well-being.
The DET OHT is in its first year of operations and rapidly developing its new, integrated approach to health care. Client and Family Centered Care; Health Equity; Diversity, Equity and Inclusion; Quality Improvement; The work of the Downtown East Toronto OHT includes focused initiatives and planning to improve care for our priority populations (People Experiencing Homelessness and Mental Health and Addictions) ensuring community members are meaningfully part of all of our work, co-designing change ideas, developing digital solutions, managing privacy and information management, strengthening governance, supporting performance and quality improvement, and developing a strategy for the OHT.
Working under the supervision and guidance of the DET OHT Manager of Operations and Population Health, the Project Manager and Advisor, Primary Care Access and Health Equity will be responsible for the successful management, execution, and completion of two strategic projects: implementation of primary care attachment project and piloting race-based data collection project. Membership in Healthcare of Ontario Pension Plan (HOOPP).
An extended healthcare and dental package.
The Project Manager and Advisor, Primary Care Access, and Health Equity, will work with Working Group Chairs, the Executive Director and Manager of DET OHT Operations to ensure the successful completion of projects related to advancing DET OHT’s Strategic Priorities, specifically focussing on:
Implementing new pathway to increase access and attachment to comprehensive team-based primary care services for target population, and
Piloting race-based data collection in one of the DET OHT projects
Implementing New Pathway to Increase Access/Attachment to Comprehensive Team-Based Primary Care Services for Target Population
For the first priority, the Project Manager will work with DET OHT’s primary care provider organizations, community members, the Primary Care Attachment Steering Committee, other health and social service provider organizations, and the DET OHT Operations Team to support the DET OHT in providing expanded comprehensive wrap-around primary care services in a target approach to connect DET residents who are lacking a primary care provider and are facing significant financial challenges, including but not limited to those experiencing housing instability, food insecurity, and other forms of marginalization, as well as those hindered from accessing comprehensive team-based care.
With supervision and guidance from the DET OHT Manager of Operations and Population Health, the Project Manager, will assume all project management and analytical duties to support the successful launch and implementation of the aforementioned initiative, including but not limited to: development and training of the new inter-professional team; data collection, monitoring, and evaluation; outreach efforts; quality improvement; community and partner engagement; Equity in healthcare access will be a central focus of their efforts across all these areas.
Piloting race-based data collection in one of the DET OHT projects
For the second priority, the Project Manager will support the newly established Health Equity and Equity, Diversity, and Inclusion (HE &EDI) Working Group to pilot race-based data collection among one of the DET OHT initiatives. As such, the Project Manager will be responsible for all project management and analytical duties related to the successful launch of the pilot including but not limited to: assessing readiness of DET OHT initiatives; building community accountability into existing structures; piloting race-based data collection; For those two projects, the Project Manager will be responsible for all administrative and operational aspects, including but not limited to defining and overseeing the scope, deliverables, and timelines of projects; identifying and managing risks and issues; conducting research and data collection, creating presentations, and completing reports as required by the OHT, Ministry or Ontario Health. In addition, the PM and Advisor will be required to engage stakeholders including but not limited to service provider organizations, community members, clinicians, DET OHT Partner Organizations (Team Members) and others.
The responsibilities of the Project Manager and Advisor, Primary Care Access, and Health Equity for the Downtown East Toronto OHT. includes:
PROJECT MANAGEMENT
Lead Working Group to develop and implement annual work and project plans.
Perform qualitative and quantitative data analysis as needed to advise on next steps.
Use project management principles and tools to ensure success of project.
Identify, monitor, and mitigate any risks to the any deliverables and the overall project.
Work with operations team to ensure the project is leveraging and connected with other projects in the DET OHT
Ensure that project objectives and KPIs are aligned with DET OHT strategic plan and OH/Ministry deliverables.
Create/complete reports and/or presentation to Core Group, other DET OHT Working Groups, Ministry, OH etc to communicate and demonstrate progress of projects.
Ensure strong relationships among project partners including family and client advisors, OHTs, OH, community agencies etc, including engaging with new interested organizations on behalf of the working groups.
Work with communications network/specialist to develop communication strategy and messaging to stakeholders.
Oversee all aspects of the management of the project.
Create and update project plan.
Ongoing identification and management of risks to projects
In this case, the incumbent will decide by consulting with the Manager of Population Health and Operations and/or the Executive Director
Finalization of the project plan
Escalation of risks to deliverables as required.
People Leadership and Relationship Building: ability to plan, organize, coordinate, integrate and motivate to achieve successful performance results. Program Management: ability to create, plan, organize, coordinate, and integrate system level programs and initiatives that meet performance targets and enhance the services provided to the OHT’s attributed population. Ability to assess risk areas and recommend mitigation solutions. Ability to initiate and oversee program evaluations and quality improvement initiatives. Ability to manage and plan annual budget expenditures. Organizational Planning: ability to lead streamlined processes to make them more efficient, and work with others to develop and adopt new procedures and processes. Community Engagement: work closely with Engagement Specialist to foster and build strong relationships with community and funding partners, as required.
Commitment to cross-agency integrated program planning: ability to work co-operatively and collaboratively within the Operations Team, management team, working groups, or in regional/provincial work groups/ committees. This includes contributing fully and actively to team activities, projects, and program initiatives to achieve group and OHT goals. Take steps to help others resolve the conflict and maintain trusting relationships. Actively champion and participate in regional and provincial initiatives that focus on diversity, inclusion and equity and embed anti-discrimination principles in programming and service delivery approaches to improve client care and staff development.

Minimum of 5 to7 years of demonstrated leadership and management of complex multi-stakeholder projects, including but not limited to, system transformation, preferably in a hospital, community health or other healthcare setting.
Strong knowledge of and experience leveraging design thinking principles and lean methodology is an asset.
Certification of Project Management Principles is an asset.
Demonstrated excellence in interpersonal, written, and verbal communication skills including, ability to demonstrate tact and diplomacy, manage confidential information, and build rapport and positive working relationships.
Demonstrated understanding of the health sector, including OHTs, primary care, health equity, acute care, home care, long-term care, and community support services
Demonstrated advanced computer skills (Microsoft Office – Teams, PowerPoint, Word, Excel, Planner, Virtual Meetings – Zoom, Microsoft Teams)
