The goal of Streets to Homes is helping homeless people and those living on low income find and keep appropriate housing, and prevent evictions, whenever possible. This is accomplished by working with individuals with mental health and/or substance use issues, within a harm reduction approach, in groups and one on one, to support individuals to develop their capacity in activities related to their daily living, and successfully maintain their housing in the Streets to Homes program.

What You Will Do
  • Undertake needs assessments and develop case plans with individuals in the program.
  • Provide case management services; assessment, personal goal setting, short and long-term planning, informal counselling, instrumental/practical assistance, follow-up supports, crisis intervention, care/service co-ordination, case conferencing, and referrals.
  • Organize, lead, or facilitate groups and activities in response to tenants’ needs which will promote life skills, independent living, and socialization.
  • Plan, develop, implement, and evaluate programming.
  • Engage tenants in development of programming and support ongoing tenant input.
  • Promote participation in recreational, psychosocial and/or clinical programming.
  • Enhance programming capacity through targeted outreach and engagement with external/community providers to increase on-site programming and/or supports.
  • Develop and expand peer support programs to support independence, skill building, and leadership development on site.
  • Work with housing staff and tenants on strategies that support tenants to maintain their housing and transition plans to support move-ins/move-outs.
  • Respond effectively and professionally to clients, situations, and emergencies.
  • Maintain all records, statistics, and forms regarding visits and program/activity enrollment.
  • Create and maintain client files in a timely manner as per WoodGreen policies and procedures.
  • Ensure targets set with Streets to Homes are met, monitor the housing challenges and ability of each resident to adhere to the policies/protocols, and develop a team strategy for intervention when problems occur in accordance with housing first and eviction prevention principles.
  • Mentor, train, and support placement students and volunteers.
  • Develop good relationships, ensuring regular communication with all tenants in the building.
  • Support with coordination of pest control for the building.
  • Engage in training and education to upgrade skills to fulfill the responsibilities of the position.
  • Keep current about the range of services available with the organization and other agencies.
  • Take part in team/unit meetings, and promote the healthy functioning of the team/unit.
  • Participate in regular supervisory sessions and annual performance appraisals.
  • Ensure the anti-racism objectives and community development principles of the organization are achieved in the program.
  • Familiar with the organization’s policies/procedures and site-specific emergency procedures.
  • Represent the unit on internal staff committees and inter-agency initiatives.
  • This position is subject to exposure to second-hand smoke, odors, pests and clients’ potential use of inappropriate language or swearing.
What You Bring to the Team
  • Bachelor’s degree in Social Work, Social Service Work Diploma, or a related discipline.
  • 3+ years related experience in mental health and harm reduction, housing help, and working with those who have experienced homelessness.

  • Experience in case management, informal counselling, and crisis intervention.
  • Strong housing help skills and ability to support individuals to incorporate a harm reduction strategy in obtaining and maintaining their housing.
  • Able to work independently with minimal supervision.
  • Exercise good judgment, flexibility, creativity, and sensitivity to changing needs.
  • Ability to minimize barriers through service delivery for low income individuals with mental health and substance use issues.
  • Ability to develop, implement, and evaluate community needs and follow up programs.
  • Interpersonal skills, both verbal and written, and ability to facilitate workshops/presentations.
  • Demonstrated information and referral skills; effectively link client to supportive services.
  • Ability to work from a client-centered approach with strong assessment skills.
  • Understanding and knowledge of the health sector and its related services.
  • Excellent knowledge of community resources.
  • Demonstrated ability to follow policies, procedures, and adhere to legislation.
  • A thorough understanding of clinical practices and care models.
  • Applied computer skills (MS word, excel, and Internet, case management software, Pirouette)
