**The Clinical Projects Specialist (CPS) opportunities have a focus in different clinical and program portfolios: Reporting directly to the Manager, Clinical Quality and in collaboration with the clinical leadership team and Directors, the Clinical Projects Specialist (CPS) projects provides oversight and leadership to Mosaic Primary Care Network clinical projects and quality initiatives and supports the implementation of program, portfolio and organizational projects and improvements within two clinical portfolios: Projects may also include support of work plans, quality plans, program planning and evaluation activities, clinical education and development, general business writing, and change and stakeholder management.
The CPS possesses advanced communication (both verbal and written) and coordination skills and is a key contributor in the planning, implementation, improvement, and sustainability of Mosaic PCN clinical service delivery enhancement and risk mitigation. This position requires the consistent application of an evidenced based approach and consistently demonstrate clinical thinking, complex problem solving, relationship building, facilitation, appropriate risk identification and escalation, and a high level of tact and diplomacy.
Mosaic Primary Care Network (MPCN) centralized MDT programs focus on providing supportive multidisciplinary services in a primary care environment for patients who self-identify as either not having (unattached) a family physician or nurse practitioner primary care provider or as having (attached) a formal relationship with a Patient Medical Home (PMH) in the MPCN catchment area.
Programs within this portfolio include a behavioral health program and a nursing and allied health program. Behavioral health programming is delivered by Social Workers, Mental Health Therapists, and Community Health Navigators. Nursing and Allied Health programming is delivered by a team of RN’s, Kinesiologists, Registered Dieticians, Physiotherapists, and Pharmacists.
The Mosaic Patient Access & Attachment (PAA) portfolio focuses on providing temporary support, transition, and attachment services to patients who self-identify as either not having (unattached) a family physician or nurse practitioner primary care provider or as having (attached) a formal relationship with a Patient Medical Home (PMH) in the MPCN catchment area.
The goal of the PAA portfolio is to stabilize unattached and vulnerable patients and facilitate their formal attachment to a Mosaic Primary Care Network (MPCN) member provider with the support of Mosaic PCN Nurse Navigators (MPCNN). PAA services aim to address the research-proven benefits of consistent access to a primary care provider, thereby improving health outcomes. The expected benefits of the PAA program include improvements across all quality domains, including increased successful attachments of unattached patients to a MPCN patient medical homes, long-term reduction in health system costs and healthcare utilization.
Project Planning and Implementation
Leads the planning and implementation of priority Mosaic PCN clinical initiatives.
Sequences and prioritizes project deliverables to achieve results on time and within the scope of the project; supports the project team in the prioritization and completion of project tasks.
Develops and implements project tools and resources as required.
Effectively engage stakeholders across a wide sector of health professionals, service providers, administrative staff, and community partners.
Communicate project goals and build commitment amongst stakeholders using innovative project and change management strategies.
Identify barriers and opportunities for the implementation of projects, including the integration of IT and privacy where required.
Plans and facilitates project meetings.
Supports and facilitates project related education where required.
Identifies, escalates risk appropriately and participates in risk mitigation and management.
Acts as an expert resource to the project team.
Provides support to project team both on the ground and virtually as appropriate.
Supports the orientation of new project team members where required.
Collaborates with the leadership team and project management team members to support integration and contextual understanding across portfolios.
Facilitates, coordinates and supports change management and stakeholder management as it relates specifically to clinical projects.
Facilitates, coordinates, and supports the implementation of education initiatives attached to clinical projects.

Education & Licenses
~ Baccalaureate degree in health-related profession or related field. A master's degree is an asset.

Experience leading projects within a multidisciplinary health care setting.
Experience with policy, guideline development an asset.
Previous leadership or supervisory experience is considered an asset.
Advanced oral and written communication skills, including interpersonal and relationship building skills.
Requires minimally and intermediate knowledge of Microsoft Suite (Word, Excel, PowerPoint, Visio, and Outlook).
Requires advanced knowledge and skill in planning.
Working knowledge of Electronic Medical Records and clinical IT systems is an asset.
Limited evening and weekend hours may be required.
Ability to travel on public transportation or valid drivers license with access to own car in good repair is required.
Willingness to work out of a number of sites, including non-conventional sites.
Maintains patient privacy and confidentiality by understanding the Health Information Act (HIA) and the Privacy Information and Protection Act (PIPA) and their applications in a clinical office setting.
