Community Pathfinder – Northern Zone
NSHPCA’s Healing Pathways Community Hub Organization is seeking a dynamic and passionate individual to join us as a Community Pathfinder – Northern Zone .
The ideal candidate will be responsible for working directly with communities, local leaders, non-profit organizations in the local and provincially, as well as Nova Scotia health system partners, to capture, report on and assist in coordinating responses to identified community needs. Community Needs Analysis:
Meet with local representatives from local communities, the health system, non-profit organizations, schools, religious groups, diverse cultural communities, and others that are active in Northern Zone.
Assess and analyze community needs for support and services in grief, loss, bereavement and emotional well-being.
Assist in reporting on demographic trends and identified community needs to coordinate with health system partners in planning appropriate responses.
Develop and maintain a database of service providers, public information and available resources, including grief training for community leaders / teachers / volunteers / others, grief literacy, and other assets available in the health system, from community groups, healthcare professionals, and in the public domain (e.g. books, videos, articles, etc.), Provide ongoing support and motivation to communities, ensuring a positive and fulfilling experience working with the Community Hub Organization.
Identify and establish partnerships with local non-profits, schools, businesses, and government agencies to improve access and development to needed supports and services for grief, loss and emotional well-being.
~Coordinate community events, information sessions, and other activities related to grief, loss, bereavement and emotional well-being.
Community Engagement:
Organize and lead informational sessions, workshops, and orientations to educate the community about information and resources available through the Hub
Develop communications materials and utilize social media platforms to promote Hub resources, funding opportunities and gain additional users for our Hub technology supports.
Act as a local liaison between our organization and the community, addressing inquiries, feedback, and concerns related to community needs, resource availability and other issues.
Promote access and awareness of community funding programs, and support non-profit organizations / charities to submit applications for funding, especially to define and measure expected outcomes.
Establish metrics and evaluation tools to measure the impact and effectiveness of community funding programs and other initiatives.
Collect feedback from partner organizations to report on outcomes achieved, and continuously improve and refine the Hub’s coordination and resource development activities.
Administrative Tasks:
Maintain accurate records of community needs, service providers, community participation, funding applications received/granted, and expected outcomes for funded projects.
Assist with other administrative tasks as assigned.
Bachelor's degree in social work and or mental health related field. Individuals with trauma-informed training are preferred.
Previous experience in resource coordination, community outreach, and/or nonprofit work preferred.
Proficiency in Microsoft Office Suite.
