**REPORTS TO**:Clinical Service Manager **EMPLOYEE GROUP**:Non-Union **HOURS OF WORK**:35/Week

**LOCATION**:Haldimand and Norfolk

**QUALIFICATIONS**:

- Diploma in Social Service Worker, Addiction Studies, Concurrent Disorders, or equivalent
- Two to Three years’ related experience, with an emphasis on working with vulnerable populations and/or community engagement
- Minimum two years experience with data collection and analysis
- Competency in Standard corporate software (Microsoft Office - Word, Excel, Outlook), capabilities in data entries and searches
- Excellent interpersonal and organizational skills
- Strong time management skills and ability to communicate effectively
- Critical thinking and problem-solving skills
- Professional verbal and written communication skills
- Ability to work independently and collaboratively as part of a multi-disciplinary team and other agencies in the community
- Basic understanding of all relevant legislation including PHIPA

**RESPONSIBILITIES**:
(Duties include, but are not limited to)
- Engage with the community and vulnerable populations to build bridges and break down barriers to access to service
- Acts as an advocate and support for those individuals being referred to appropriate community agencies
- Maintains documentation and records in accordance with both the provincial legislation and agency policy
- Supports service delivery team with participant follow up
- Maintains strong participant confidentiality practices and professional boundaries
- In collaboration with the service delivery team, assists with referral of participants to appropriate mental health, social, education, employment, medical and other resources and completes follow-up on referrals
- Assists with the creation and distribution of outreach materials in the community
- Ensures documentation is complete, including files, case notes and reports using the agency’s electronic records system
- Maintains accurate statistics and drafts reports as required
- Travel required daily and at times, may be in adverse weather conditions
