Registered Nurse- Behavioural Supports Ontario

Purpose:
To provide holistic nursing care in accordance with the College of Nurses of Ontario, organizational and unit standards, as well as participate and collaborate with other health care providers in provision of patient care in the community. Responsible for making clinical decisions and exercising judgment to ensure the best possible nursing care is provided within available resources.
DUTIES:
Effectively use the Standards of Nursing Practice and the nursing process to assess, plan, implement, and evaluate patient care of the senior with mental illness and/ or behavioral disturbances Assist in the development and implementation of standardized protocols to assist the individuals with responsive behaviours of those in the community and in LTCH. Make recommendations for the care of older adults with challenging behaviours brought on by mental health, addictions, dementia, delirium and neurological conditions Assist in the implementation of these recommendations reducing the likelihood of hospital admissions Participates collaboratively with other health care providers in the provision of care Liaises with the psychiatrists, family physicians, LTC community and other care providers to ensure continuity of care and to reduce lengths of stay in the hospital. Works in collaboration with the BSO Integrated Response Team in the district of Algoma Participates in data collection for statistical, Ministry and research purposesQUALIFICATIONS:
Current registration with the College of Nurse of Ontario in good standing is required. A valid driver’s license and full time use of reliable vehicle is requiredEXPERIENCE:
Minimum of 3 years of recent applied clinical experience or equivalent working with older adults with or at risk of responsive behaviours presenting with mental health, dementia, delirium, substance use and/or other neurological diagnoses and their care partners is required. Advanced knowledge & demonstrated practical clinical application of geriatric mental health and specialized dementia care including pharmacological and non-pharmacological best practice principles within the RN scope of practice is required.SKILL/ABILITY:
A proven ability to liaise with referral sources and other community agencies required Excellent understanding of community resources Knowledge in evidence-based best practice in dementia, delirium, mental health, and substance use Knowledge in PIECES, GPA, U-First, and Montessori practices Proficient in computer skills including the use of Microsoft, Meditech and other applications Demonstrated teaching, research, leaderships and organization skills Demonstrated ability to maintain successful working relationships; ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner, both in person and on the telephone; tact, courtesy and alertness in public contacts Demonstrated behaviour consistent with the Hospital’s mission, vision, and iCcare way-
