Reporting to the Manager of the Nurse Led Outreach Team (NLOT),with a relationship with the Palliative Care

physician’s group will work to develop and establish the role ofPalliative Care Transition and Outreach Nurse

Practitioner. The Palliative Care Clinical Coach and OutreachNurse Practitioner will be an individual with an

understanding of palliative care, including complex symptommanagement and illness trajectory, as well as

understanding the complexities of system navigation in the city ofThunder Bay and the region served by TBRHSC.

The position is responsible for working with participatingcommunity organizations in alignment with provincial

direction established by Ontario Health and guidance developed bythe Ontario Palliative Care Network. The

Palliative Care Clinical Coach and Outreach Nurse Practitionerwill drive practice change, with a focus on improving

access to and quality of clinical care, aligned to the PalliativeCare Health Services Delivery Framework in the

Community and the Ontario Palliative Care Competency Framework.

The Palliative Care Clinical Coach and Outreach Nurse Practitionerwill:

• Provide palliative care coaching and mentoring to staff andclinicians in participating community organizations, to

build primary‑level palliative care competencies.

• Collaborate with leaders of community organizations on localservice integration, and change management

initiatives, using quality improvement methodologies to achieveresults.

• Consult on clinical care when the complexity of the patients’needs is beyond the existing competencies of providers

(in the community organizations).

• Provide support to Indigenous communities and Indigenousorganizations for their implementation efforts, where

Indigenous communities wish to be involved, in collaboration withregional partners

The role will consist of clinical work with patients with a lifelimiting illness and their families as well as coaching with

all members of the care team. The goal of coaching is to enhancethe capacity for robust palliative care in the

institution and in the region. The six domains of clinicalpractice that will be supported by the position: Clinical

Expert, Consultation, Change Agent, Leadership, Research andEducation. The time dedicated to each of these

domains will be based on the current program needs and will bevariable during the course of program development.

EMPLOYMENT REQUIREMENTS:

Education/Experience:

* Current registration as a Registered Nurse in the Extended Classwith the College of Nurses of Ontario

* Successful completion of a Master's Degree preferred

* Current Basic Cardiac Life Support (BCLS) certificate isrequired

* Active Professional Liability Coverage required

* Membership with RNAO or NPAO is preferred

* Minimum of 3 years recent nurse practitioner experience duringthe last 5 years is preferred

* Advanced training in palliative care preferred, e.g. LearningEssential Approaches to Palliative Care, Hospice Palliative Care NursingCertificate, or equivalents

* Experience in the development, implementation and evaluation ofrelevant advanced nursing practices is required

* Experience in the development of strategies to build capacityand improve health outcomes is required

* Experience in the development of clinical guidelines andprotocols, promoting the use of research and

evidence‑based practice to guide patient‑centered careis required

Skills/Abilities:

* A commitment to developing equity‑based, evidence‑informedapproaches for delivering palliative care services for all people in Ontario, regardless of theirage or illness

* A good understanding of regional priorities and local healthcare needs, within Ontario’s health care landscape

* Ability to work well under pressure and use good judgment inassessing difficult situations.

* Ability to work independently and as part of a team

* Aptitude for proactive problem solving, using strong criticalthinking, and negotiating skills.

* A demonstrated ability to develop, deliver and evaluate trainingand coaching efforts

* An ability to develop relationships based on trust, compassion,and empathy

* Strong active listening and interpersonal communication skillsfor one‑on‑one coaching

* Experience designing education to effectively address palliativecare training needs of inter‑professional learners

* Excellent communication skills and experience with culturallyresponsive teaching

* A demonstrated focus on person‑centered care

* Supports the advancement of equity, inclusion, and diversityacross Ontario's health care system.

* Commitment to engage with First Nations, Indigenous, Métis andurban Indigenous people as they self‑determine their own approach to palliative care

* Proficiency in educational technologies (e.g., webinars, apps)as well as knowledge of Microsoft Outlook, Word, Teams, PowerPoint, and Excel

* Access to reliable and efficient mode(s) of transportation toenable travel between community organizations. Effort‑ Physical/sensorydemand

Assets:

* Certificate or willingness to work toward completion ofCertificate in Palliative Care

* Experience in assessment and management symptoms in PalliativeCare

Salary Range: $65.210 to 70.750
