The Opportunity

Reporting to the Manager of Hematology/Oncology, Nephrology, and PACT, the Resource Access Navigator practices as a member of an interprofessional team to provide resource navigation services and family-centered care with children, youth, and their families within the Hem/Onc/Neph teams. The Hem/Onc/Neph Resource Navigator will connect families to health center and community-based resources to enhance wellness related to the social and structural determinants of health. The Navigator tasks will include (but are not limited to) assisting in enrollments, submissions, renewal requests, appeals, etc for patients and families applying for Pharmacare, cancer programs, private insurance claims, patient support groups, drug funds, other options for support with medication and equipment costs. The Navigator works in collaboration with nursing, physicians, pharmacists, social workers, and other health care providers in the Hem/Onc/Neph teams.  Other responsibilities include liaising with a range of health, community, and government services with an emphasis on building collaborative partnerships, facilitating resource acquisition, and advocating for resources to meet the client’s needs to optimize their health and wellness. The Resource Navigator may have responsibilities in program development, program evaluation, and quality improvement as needed. The Resource Navigator serves as a resource to other interprofessional team members on a direct and/or consultative basis as needed. 

Hours of Work

7.5 hour shifts (between 0800 and 1900) Monday - Friday.

Your Qualifications

• Bachelor's Degree in social work required (other health care professional degrees with experience in supporting families, navigating community supports, and accessing funding may be considered).
• Current license, or eligible for licensure, with the Nova Scotia College of Social Workers (NSCSW) required (or appropriate licensing body if another degree).
• Minimum of two (2) years related clinical experience required.
• Experience working with patients and families with hematology, oncology, and nephrology conditions preferred. 
• Knowledge of IWK and community resources and programs (NB, NS, and PEI), insurance plans, out-of-province coverage, and drug funding and skills in navigating those systems.
• Knowledge of supports required and resources available to Newcomers for medical needs.
• Knowledge of the Child and Family Services Act and other relevant legislation required.
• Experience in interviewing families, case management, advocacy, and assisting with funding applications required.
• Demonstrated ability to coordinate the procurement of medical equipment supplies, and services for patients and families.
• Demonstrated ability to reach a collaborative care plan with the health care team, patient, and family.
• Demonstrated excellent time management, critical thinking, organization, decision-making, and problem-solving skills required.
• Demonstrated exceptional interpersonal, presentation, and communication (verbal and written) skills required.
• Experience in Meditech, MS Outlook, and MS Office Suite (Word, Excel, PowerPoint, etc.) preferred.
• Competencies in languages other than English an asset.
