The work you will do: Visit homes and long-term care facilities to provide treatment and support for residents as needed Monitor and manage stable conditions through regular in-home visits, offering daily or weekly follow-up care Conduct comprehensive in-home assessments and deliver ongoing care for patients who are unable to attend clinic appointments Why Bayshore? Experience Required A minimum of 3,000 hours within the past 5 years as a Home Care Nurse OR 1,500 hours within the past 5 years providing care in remote, isolated, or semi-isolated communities as a Home Care Nurse. #J-18808-Ljbffr
