Integration Lead, Mental Health and Addictions
Fulltime Position (18 month contract), Starting January 2025
Wage: remuneration of $42.00 to $52.00 per hour depending on background
The Mental Health and Addictions Integration Lead is responsible for advancing the development of a Regional Integrated and Coordinated Mental Health and Addictions System for Brant, Haldimand and Norfolk regions along a continuum of care that includes acute, community mental health and addiction services, and Primary Care. This position is an employee of the Canadian Mental Health Association Brant Haldimand Norfolk (CMHA BHN) that is seconded to the Brantford Brant Norfolk Ontario Health Team (BBNOHT). For more information on the BBNOHT, visit As such, the position will have accountability to the Director/Executive Lead for the BBNOHT.
The Integration Lead will work closely with the leadership of the local health care providers to advance an integrated system that delivers high quality mental health and addictions services and improves the health status and health outcomes of those needing this care. This position will provide leadership, direction, coordination and management in the development and implementation of a model of care that streamlines access so that patients and clients can seamlessly transition between levels of care depending on their care needs. CMHA BHN as a local health service provider that crosses Brant, Haldimand, and Norfolk Counties is the host organization for this joint venture.
The Integration Lead will be expected to:
1) Work closely with the regional Mental Health and Addiction providers, Primary Care, area hospitals, Indigenous service providers, community partners, and other identified priority populations.
2) Develop and submit monthly and quarterly reports for OHTs and Ontario Health West, as required.
3) Support the BBNOHT MH&A Steering Committee to develop and achieve projects which are aligned with the BBNOHT Strategic Plan.
4) Support and lead quality improvement projects at the OHT level and also within CMHA BHN.
5) Contribute to any regional communication strategies from a MH&A lens.
6) Liaise with other members of the OHT Secretariat and leadership structure as required to implement complex projects.
7) Review and analyze mental health and addiction services to identify where resources could be restructured to support specific projects or tasks to advance a population health management approach.
8) Facilitate and provide leadership at meetings whose agenda is to advance the overall project goal.
9) Develop and maintain a master plan of deliverables which includes any related sub-project plans.
10) Ensure the project goal aligns with all specified requirements and timelines.
11) Support other analysis as required.
12) Participate in applicable education sessions such as Indigenous Cultural Safety training.
Eligibility Requirements:
· Master’s Degree in Health Sciences or another Health Discipline is preferred;
· Minimum of five years direct experience in system development with demonstrated effectiveness;
· Project Management Professional designation or advanced training in Lean Six Sigma, Black Belt level is preferred;
· Reliable transportation is necessary. Travel within the geography is anticipated;

· Proven experience with engaging and developing collaborative teams of dedicated professionals;
· Exceptional interpersonal, verbal and written communication skills;
· Excellent time management, risk management, financial management and change management;
· Strong organizational and computer skills;
· Demonstrated professional leadership and/or project management skills, attributes required;
· Considerable understanding of the community mental health & addiction services sectors is preferred;
· Knowledge of local community social services preferred;
· Strategic, conceptual and analytical thinking and decision making skills;
· Evidence of updated vaccination status.

Hours of work may require some flexibility beyond regular daytime weekday hours. Depending on the project development requirements, occasional weekend work may be necessary as well as some overtime hours to complete the time limited tasks.
Work location will be a mix of remote and on-site. The position will be required to attend on-site to the various community agencies upon request. A primary office space location will be provided in the region.
