**Background**:
Ontario Health Teams (OHT) have been introduced by the Ministry of Health to improve the delivery of integrated and patient-oriented care. At maturity, OHTs will offer seamless, coordinated services across the health care continuum including acute care, primary care, home care, and community care.
The Brantford Brant Norfolk Ontario Health Team (BBNOHT) is an integrated healthcare initiative designed to improve patient care and streamline health services in the Brantford, Brant County and Norfolk County regions. Officially approved in November 2020, the BBNOHT consists of a collaborative network of over 20 health organizations, including hospitals, community care providers, primary care, and mental health and addiction services.
BBNOHT aims to provide a seamless continuum of care by ensuring that various health care providers work together efficiently. This model emphasizes communication and collaboration to enhance patient outcomes and experiences.
**The Position (1)**
The Digital Health Integration Specialist will play a key role in leading the digital integration projects across BBNOHT member organizations to support BBN OHT strategic projects which include Advancing Digital Health, Navigation, and the Inter-Professional Primary Care Team
**Key Responsibilities**
- **Strategy Development**: Contribute to the development and execution of a comprehensive digital transformation strategy aligned with Provincial, Regional (OH West) and local strategic objectives.
- **Project Management**: Participate in project management processes and provide updates (verbal and or written) to ensure deliverables are completed within the timelines identified. Work cross-functionally with OHT members and collaborators to promote alignment and coordination of priority work streams including the IPCT.
- **Change Management Strategy**: Develop and implement comprehensive change management strategies and plans for digital health initiatives. Work collaboratively to ensure that the IPCT program development includes digital health initiatives to meet program goals.
- **Invested Partners**: In collaboration with BBNOHT team, and organizations, Identify and engage partners, ensuring their needs and concerns are addressed. Facilitate communication between stakeholders and project teams.
- **Communication**: Design and execute communication plans in alignment with the BBNOHT communication plan to keep stakeholders informed about the progress and benefits of digital health projects. Assist in the development of fostering digital literacy throughout BBNOHT.
- **Education and Support**: Develop and deliver training/education programs to ensure staff are equipped to use new digital tools and processes effectively. Provide ongoing support and resources.
- **Impact Assessment**: Assess the impact of digital health initiatives on workflows and processes. Identify potential challenges and develop mitigation strategies.
- **Performance Measurement**: Monitor and report on the success of change management efforts, using data to make informed adjustments to strategies.
- Contribute to safe, quality care by managing a workload effectively in a fast-paced environment.
- Addresses and resolves issues in a productive, respectful manner.
- Exercises sound professional judgment and uses best practice guidelines.
- Contributes to a safe, inclusive environment for all through compliance with patient and staff safety policies and procedures.
- Ensure compliance with healthcare regulations, data privacy laws and industry standards
**Skills**:
- Bachelor’s degree in healthcare, administration, business administration, information technology, or a related field.
- Clinical background and or Information Technology / Information Management experience an asset.
- Certification in change management (e.g., Prosci) or project management (e.g., PMP) an asset.
- Minimum of 5 years experience as an analyst, informatics role in digital health, informatics, healthcare IT or healthcare transformation roles
- Strong understanding of healthcare systems, clinical workflows, and health data analytics
- Excellence communication, interpersonal and leadership skills
- Strong problem-solving abilities and strategic thinking
- Knowledge of healthcare regulations, including HIPAA and data privacy standards
- Experience with measurement and evaluation of program outcomes.
- Enthusiastic, creative, and willingness to learn.
- Proven ability to work collaboratively with multiple team members and demonstrate effective change management experience.
- Superior analytical and problem-solving abilities.
- Strong written, verbal communication, facilitation, and interpersonal skills.
