As a Case Manager, your mission will be to assist clients in navigating their housing journey with a strong emphasis on Inuit culture and community values. You will work closely with individuals and families, helping to create individualized service plans while advocating for their rights and needs. Strong organizational skills and a commitment to client-centered care are essential for success in this role.
Key Responsibilities: • Deliver services with professionalism and cultural awareness • Create and implement risk minimization plans • Evaluate client progress and adapt services accordingly • Maintain organized case files based on TI's standards • Represent TI effectively within the community
Requirements: • Diploma in human services or social work • Relevant housing service experience, preferably with Inuit communities • Proficient with Microsoft Office and digital communication tools • Training in first aid and crisis de-escalation • Excellent communication and teamwork skills
Contribute to meaningful change in the lives of individuals facing homelessness through effective housing case management at Tungasuvvingat Inuit.
