Manager, Transitions ProgramsReporting to the Director Client Care, the Manager Transitions Programs ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, community support services, primary care and acute care teams. Together, they create personalized plans that improve outcomes and ease the transition to home. The Manager, Transitions Programs provides operational leadership to transitional programs and the interdisciplinary teams that will be providing holistic care to the clients in the community. You are passionate about informing and shaping transitional programs from hospital to the community and being at the forefront of healthcare innovation in Ontario. You will provide exemplary leadership and operational management for interdisciplinary team(s) including nursing, therapies, social work, dietitian, PSWs and community support services care providers. You will ensure excellence in the provision of client care, the achievement of corporate/program objectives; planning and operations; quality management and improvement; participation in special projects, corporate/research initiatives and other activities that support and advance organization goals. This role will have accountabilities in operational and professional practice excellence; creating a healthy workplace that maximizes the potential of all members of the team. What you will do:Provide clinical practice leadership, coaching and oversight management to ensure the safe, effective and efficient provision of care excellence to all clients and family members for transitions programs.Develop, implement, manage and review short- and long-term plans for the clinical area(s) which are in aligned with organizational goals and strategies; identifies and prioritizes time sensitive initiatives.Develops and implements day-to-day operating policies, systems and procedures to contribute to organizational effectiveness, quality improvement, and innovation.Develop staff utilization plans to ensure adequate level/mix of human resources to execute plans and achieve required outcomes.Identifies and manages employee issues/concerns.Manage, motivate and provide professional/clinical leadership to interdisciplinary teams; ensuring ongoing practice and professional development of a cohesive team;Apply structured change management methodologies (e.g., ADKAR, Kotter's 8-Step, etc.) to guide transitions and foster engagement.Mentor clinical staff and contribute to workforce development initiatives.Collaborate with the Professional Practice and human resources to ensure proper onboarding of new client care employees.Promote external and internal sharing of best practices to increase efficiency, reduce costs and improve quality.Implement oversight mechanisms to monitor, measure, and evaluate compliance with regulated standards of care. Conduct regular performance appraisals, in a manner that is fair and consistent.Lead by example to plan and implement continuous quality improvement and other initiatives in the clinical area(s) by creating an environment where innovation and creativity are encouraged. Addresses adverse events, complete risk investigation and develop risk management strategies.Adopts Diversity, Inclusivity, and Equity in the delivery of client care services.On-call as required for programs supportWhat you bring:Registered healthcare professional licensed to practice in Ontario and member of one of the regulatory colleges such as but not limited to the professions of RN, OT, PT, SW, DT, Kinesiologist, Psychologist, Behavioralist; required.Undergraduate preparation in a Healthcare related Discipline, required.Master's prepared; assetMinimum 3–5 years of experience in management; preferred.Knowledge of the related regulatory and legislative requirements within Ontario; required.Demonstrated business acumen to development and implementation new ventures.LEAN Greenbelt, PROSCI, or PMP certificate; asset.Demonstrated experience in change management, project leadership, and interdisciplinary collaboration; asset.Proven understanding of clinical service delivery in a homecare environment.Excellent communication and leadership skills.Good command of Windows, Microsoft Office 365 (Outlook, Word, Excel, Teams, etc.).Experience supporting quality and risk management initiatives.Demonstrated ability to work independently and to take initiative.Understands and uses effective communication methods/tools adopted by the organization to communicate effectively influence change, direct and motivate others.Adapts to changes in the role by acquiring the appropriate knowledge and skills Ability to promote and facilitate data driven, evidence-based practice decision-making within a community setting with demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams and drive results.Demonstrated effective critical thinking, analytical, and problem-resolution skills. Maintains the level of competencies and skills required to support staff.Excellent organizational, planning and time management skills. Thrives in a fast-paced environment.A vehicle and valid driver's license – and the ability to travelBilingualism (French/English); asset.What we offer you:The hiring range for this position is $95,101.50 to $114,016.50 per year, which reflects the expected starting salary based on qualifications and experience. Please note, this is not the full salary range for the position.
