Position Summary

The Outreach Worker will function as a member of the Mental Health Interdisciplinary Team and as such will be involved with cases of a complex clinical nature. Working under the supervision of clinicians in the Case Management Program, and Adults Program and Adult Counselling Program, the Outreach worker will be responsible for: assessing and evaluating care and treatment plans and discharge plans: implementing care plans, providing crisis intervention; participating in team conferences; staff meeting and professional development activities; maintaining appropriate documentation in electronic health records and monitoring program statistics; group facilitation; participating in the planning and evaluation of the MH program and performing other duties as required.

The Mental Health program is currently seeking a Permanent Part-time Outreach Worker.

Essential Qualifications

  • Must be registered with a regulated professional body
  • Social Service Worker 2 year diploma, or a health care related diploma/degree with three (3) years Mental Health experience
  • Direct experience in Mental Health field, preferably in community outreach working with clients experiencing mental illness
  • Experience in group facilitation
  • Non-Violent Crisis Intervention or Trained in CPI
  • Knowledgeable about psychosocial rehabilitation
  • Knowledgeable about services and systems for assisting people with mental illness
  • Excellent communications skills; written and oral
  • Team work skills
  • Demonstrated ability to maintain professional and collegial relationships with co-workers and other agency personnel
  • Knowledge and ability to identify people at risk of self-harm or neglect, or harm to others, and to formulate appropriate responses in the situation. (i.e. crisis management skills suited to community settings and solitary worker settings)
  • Demonstrated experience in providing home visits and ability to work independently according to safety policies
  • Excellent time management skills
  • Current unrestricted driver's license and access to reliable insured vehicle
  • Must be able to fulfill physical requirement of the job as per current Physical Demands Analysis

Preferred Qualifications

  • Demonstrated awareness of resources and referral protocols for consumers and family members an asset
  • Demonstrated ability to provide health teaching for co-morbid conditions, i.e. experience or training in Motivational Interviewing
  • Demonstrated/documented experience in facilitating educational sessions to clients, family and community
  • Sound knowledge and experience in working with clients who may have concurrent disorders

Hours of Work

Must be available for day shift, Monday to Friday. Shifts and hours may vary according to departmental requirements and as per the OPSEU Collective Agreement.
