Job Summary
The Clinical Team Lead (CTL) role is a clinical liaison and leadership (non-supervisory) position within the Psychosocial Oncology (PSO) Team, reporting to the Clinical Manager, Outpatient Oncology. The successful candidate will work within a fast-paced environment as a collaborative team member, focusing on day-to-day operational needs, standardization of work, educational support, and advancing the offerings of the PSO team. The CTL will work closely with other members in the Oncology Department to ensure patient care needs are met. The CTL will be actively involved in patient/group counselling. Interventions are facilitated through counselling, psychoeducational and therapeutic groups, advocacy, identification of barriers to treatment, resource referral, consultation, collaboration, teaching, research, and patient/family/staff education. Provision of social work services is in accordance with the mission, philosophy and values of Trillium Health Partners, the standards of the Ontario College of Social Workers and Social Service Workers. As well, social work activities are provided in accordance with associated legislation and in collaboration with Cancer Care Ontario and Canadian Association of Psychosocial Oncology.
Major Responsibilities
- Create an environment that supports and advances professional practice, teamwork and communication through mentoring and role modeling the application of Psychosocial Oncology clinical knowledge
- In collaboration with the manager, develop schedules and assignments, and address resource utilization
- Monitor productivity of the service based on data (i.e., number of consults, etc.)
- Monitor and address patient and family satisfaction with care
- Facilitate staff orientation and identify staff learning needs and appropriate resources in collaboration with program Educators & Manager
- Support the accountability of team members to the Manager with regard to patient care, professional conduct and performance
- Participate in hiring, orientation and performance evaluation of staff
- Liaise with members of the interprofessional team (inpatient, outpatient and Emergency Department) to ensure excellence in patient care
- Complete regular chart reviews, audits and follow-ups to ensure adherence and compliance to standards of care
- Support local leadership with interpersonal relationships among the interprofessional team through coaching, mentoring and facilitating difficult conversations
- Support patient flow, discharges and transfers of patients through a transparent, equitable and standardized process
- In collaboration with PFA's, Professional Practice, Oncology leadership, Decision support and the PSO team, implement a strategic vision for PSO services
Job Requirements
Education
- Master of social work degree from an accredited school of social work
Experience
- Leadership experience preferred
- Minimum of 3-5 years relevant social work experience within an acute care setting
- Minimum of 2 years recent experience in oncology and palliative care
- Experience with crisis and urgent situations
- Previous training/experience with the stages of illness that include diagnosis, recurrence, end-of-life, bereavement and survivorship.
- Systematic understanding and execution of the hospital processes and bed flow needs.
- Experience with legislation that governs public hospitals such as the Substitute Decisions Act, Health Care Consent Act, Consent to Treatment Act, Mental Health Act.
- Knowledge of Microsoft Outlook, Excel and electronic health record operating systems such as Meditech.
Professional Affiliations/Memberships
- Membership in good standing with the Ontario College of Social Workers and Social Service Workers.
- Membership in the Ontario Association of Social Workers preferred.
Additional Skills
- Experience creating/modifying practice standards and writing standard operating procedures
- Demonstrates effective clinical counselling skills with the goal of assisting/supporting patient, family, and caregivers to adjust to the effects and complications of cancer.
- Demonstrates in-depth knowledge of high risk and vulnerable patient populations.
- Comprehensive knowledge of community resources and research skills to secure new resources.
- Demonstrates strong communication and interpersonal skills with the ability to work collaboratively with patients/families, interprofessional team members, and community stakeholders through face to face or telephone interventions, class teaching, group facilitation, adult learning.
- Detail-oriented with excellent organizational skills.
- Demonstrates a high degree of initiative, self-direction and ability to multitask in a fast paced environment.
- Operating in accordance with the Code of Ethics and Standards of practice as determined by the College of Social Workers and Social Service Workers
- Strong self-care skills.
