Waypoint Centre for Mental Health Care is a 315-bed academic and teaching hospital that provides specialized mental health, addiction and geriatric care. Located on the shores of Georgian Bay, we serve some of the most complex and disadvantaged individuals in Ontario and are the sole provider of high-secure forensic mental health services in the province.

The Manager of the North Simcoe Youth Wellness Hub (NSYWH) is currently seeking a Full Time Care Coordinator. As a core member of the North Simcoe Youth Wellness Hub, this role oversees and supports the daily operations of the NSYWH. The Supervisor serves as a liaison between Waypoint, the lead agency, and Youth Wellness Hubs Ontario (YWHO), collaborating with external service providers while fostering and maintaining relationships with community partners and other relevant services.

Please Note: This position offers a hybrid work environment consisting of in-office and remote work. Office location 287 Bayshore Drive Midland ON.

KEY ACCOUNTABILITIES:

  • Prepare for, participate and provide leadership in local and provincial Youth Hubs Ontario (YWHO) meetings, committees and working groups.

  • Translate provincial expectations (e.g. core components) into actions and achievable outcomes for North Simcoe in collaboration with partner agencies.

  • Update the youth wellness team and partner agencies on ongoing changes to protocols, support and continue to evolve the stepped care pathway and intake process.

  • Provide seamless transfer of services between NSYWH and partner agencies as appropriate.

  • Oversee the clinical aspects of the NSYWH stepped care pathway and aligning the pathway with provincial expectations, including: referral pathways, screening and referral, assessment and triage of youth, treatment, discharge and documentation.

  • Facilitate continued engagement and communication with all stakeholders (e.g NSYWH, YWHO, community partners, youth) building and maintaining relationships with community partners and other relevant services/providers.

  • Work from a harm reduction, collaborative/team based and youth centered approach.

  • Create and maintain a developmentally appropriate, non-judgmental, non-stigmatizing, safe and welcoming hub atmosphere, in collaboration with youth, service providers and the lead agency.

  • Gather, analyze and provide data from the NSYWH such as volumes, population served, service offerings and needs, standardized measures etc., to inform the advancement in the provision of services.

  • Occasional participation in events outside of normal working hours.

REQUIREMENTS/QUALIFICATIONS:

  • Undergraduate degree in a Health or Social Service related field required, with a Master's degree in a related field preferred.
  • Registration with an applicable Regulatory college or professional association (e.g. College of Nurses of Ontario, Ontario College of Social Workers and Social Service Workers, Ontario Society of Occupational Therapists) is required.
  • Demonstrated understanding of local community issues impacting youth, and the needs and challenges facing diverse youth and equity-deserving youth.
  • Knowledge of community resources available to support youth and families across a continuum of care and holistic understanding of health and mental health.
  • Experience with clinical practice, team processes, risk management procedures, professional standards, and current trends and approaches to treatment and rehabilitation of mental illness in order to lead staff and to assess, implement and evaluate effective delivery of youth services and operational needs within an inter-professional, community base environment an asset.
  • Extensive knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQ+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction, and human rights and accessibility frameworks.
  • Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
  • Demonstrated leadership, conflict resolution, interpersonal and organizational skills
  • Proven written and verbal communication skills
  • Proven ability to adapt readily to change and reflects an optimistic and positive attitude
  • Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation)
  • Preference for proficiency in French/English language skills.
