Nurse Practitioner- Primary Care Access and Attachment Team

In this position, you will engage in collaborative work within the community, with: HAMSMaRT, Refuge Newcomer Health, Shelter Health Network, Canadian Mental Health Association and Good Shepherd, Hamilton as part of the Primary Care Access and Attachment Team (PCAAT). This team will be focused on connecting with vulnerable populations to increase access and attachment to Primary Care.
The Nurse Practitioner will be working within PCAAT, a community-based hub of interdisciplinary professionals providing on-the-spot care to clients, collaborating with their circles of care, and improving overall health outcomes of marginalized individuals. In this position you will be working with the team at HAMSMaRT with day-to-day operational accountability to both Good Shepherd and your Program Site. HAMSMaRT's work strives to provide a patient- and community-led, humanizing alternative health care service through centering the social determinants of health and understanding systems of power in the work being done.
The work of PCAAT is based on a fundamental belief in promoting hope and dignity to the most under-served members of our community. You will be working with our patients/clients, visitors, and staff by providing comprehensive primary healthcare services, including health promotion, disease prevention, treatment of injury and illness, consultation, and support.

How You Will Make an Impact
To work within the full scope of the RN (EC) practice to provide primary care to all individuals served by PCAAT and other appropriate social services in accordance with the College of Nurses of Ontario's Expectations for Consultations with Physicians by Registered Nurses in the Extended Class. To provide primary care as needed directly to referred individuals, those accessing HAMSMaRT services, or those connected with via outreachTo act as an important communication link between PCAAT staff and all service users and providers (e.g. physician, service recipients, community partners) to ensure effective communication and the timely sharing of information necessary to promote the health and wellness of our clients.To provide comprehensive assessments and, as appropriate, health examinations and assess the need for orders and interpret appropriate diagnostic, screening and lab tests and reports.Formulate diagnoses and communicate and interpret these to clients and family members.Provide care to well individuals of all ages and their families, placing emphasis on health enhancement and risk reduction.Engage in outreach to other services within the community which serve the target population of PCAAT and HAMSMaRTUtilize a holistic approach and a client centered approach to care, paying special attention to psychological, social, cultural and environmental factors.Utilize health promotion strategies such as education, advocacy, and caring interactions to support individuals through all aspects of their health and illnessTriage, communicate and collaborate with hospitals and other community health services (e.g. hospitals, specialists, and other family physicians).Observe all health and safety rules and work in compliance with the provisions of the occupational Health and Safety Act and its regulations.What You'll Bring to Our Team
Bachelor's degree in Nursing and Registration in the Extended Class with the College of Nurses of OntarioTwo years' experience as a Nurse Practitioner, RN, (EC) preferably in a community settingExperience working with mental health, addiction, and concurrent disordersExperience and sensitivity in working with culturally diverse populations including individuals newly arrived in CanadaCPR/First Aid certification and WHMIS training and/or willingness to certifyCommitted to intersectional anti-racist/anti-oppressive practice, harm reduction, and trauma-informed careAbility to work both independently and as a member of a teamOutgoing and adaptable to working in a fast paced and changing environment.Creativity and flexibility in achieving desired outcomes.Hospitable in dealings with clients and other key stakeholdersAvailable, respectful and responsive to the needs of othersStrong written and verbal communication skillsGood interpersonal and organizational skillsWhat We Offer
