A solo primary care physician practice located on the Hamilton Mountain is seeking a part-time Nurse Practitioner to join our friendly and supportive team. As a member of the Hamilton Family Health Team, our practice benefits from extensive interdisciplinary support, including access to a dietitian, mental health counsellor, and clinical pharmacist.

You will be supported by experienced administrative staff and one Registered Nurse, allowing you to focus on providing high-quality patient care. Our clinic uses TELUS PS Suite EMR for patient records and offers a collegial, patient-centered work environment.

Position Summary

The Nurse Practitioner will work independently to provide comprehensive primary care to a diverse patient population across the lifespan. This role is ideal for an NP who enjoys autonomous practice while being part of a collaborative healthcare network.

Scope of Practice

The Nurse Practitioner will provide care in the following areas:

  • General primary care
  • Pediatric care
  • Prenatal care
  • Well-women visits
  • Chronic disease management (including diabetes)
  • Mental health care
  • Geriatric care
  • Immunizations and vaccinations
  • Assessment and management of acute issues
  • Preventive care, health promotion, and wellness

Responsibilities

  • Conduct comprehensive patient assessments and develop treatment plans
  • Diagnose and manage acute and chronic conditions
  • Prescribe medications and order/interpret diagnostic tests in accordance with scope of practice
  • Provide patient education, counseling, and preventive care
  • Collaborate with allied health professionals within the Family Health Team as needed
  • Maintain accurate and timely documentation using TELUS PS Suite EMR
  • Practice in accordance with professional, legal, and ethical standards set by the CNO

Qualifications

  • Registration in good standing with the College of Nurses of Ontario (CNO) as a Nurse Practitioner
  • Experience in primary care is an asset
  • Strong clinical assessment, decision-making, and communication skills
  • Ability to work independently and manage a patient roster
  • Comfort using electronic medical records (TELUS PS Suite experience an asset)

Job Type: Part-time

Pay: $58.00-$65.00 per hour
