The Outreach, Engagement, and Attachment Worker supports patients in accessing primary care by building strong relationships with healthcare providers, community organizations, and patients. Drawing on their existing relationships and understanding of the health care, social services and other related systems, this individual will collaborate to develop and implement strategies to address barriers to care while aligning efforts with the strategic goals of the Hamilton Family Health Team and the Greater Hamilton Health Network.
Key Responsibilities…
- Collaborating with health and community organizations to identify underserved populations and create pathways for attachment to primary care.
- Engaging with patients, community agencies, and healthcare and act as a liaison and navigator between the patients and providers to ensure clear communication and trust-building.
- Attending and actively participating in community and outreach events to engage with stakeholders and promote healthcare attachment services.
- Engaging with the public to identify healthcare needs and barriers to accessing primary care.
- Providing personalized guidance to patients regarding attachment to primary care; helping them navigate and advocate within healthcare systems and connecting with appropriate primary care providers.
- Address social determinants of health by connecting patients with relevant community resources.
- Assisting in the provision of ethical verbal hand offs/transfers of accountability to facilitate a successful attachment.
- Taking a patient-centred approach when dealing with challenging situations – advocate, intervene and provide patient centered transitions to primary care as required.
- Collecting, understanding and presenting data related to patient attachment rates, primary care availability, and community needs.
- Conducting outreach to community organizations and healthcare providers.
- Facilitating regular communication with community partners to strengthen collaboration.

Qualifications…
- Post-secondary education in Social Services or related field. Registration with regulatory college considered an asset.
- Experience in healthcare or community services, preferably within the Hamilton area.
- Strong relationship-building, interpersonal, communication, and problem-solving skills.
- Knowledge of healthcare systems, and the social determinants of health.
