GCSN acts as the "front-door" for crisis services for adults (16+) living with a developmental disability or dual diagnosis in the City of Toronto. Through a collaborative network of formalized partnerships with cross-sector service providers, the GCSN provides time-limited crisis and transitional supports including case management, and access to generic & specialized services including short term day programming and residential safe beds. The GCSN also works collaboratively with DSO-Toronto Region to provide "Urgent Response" supports to individuals and families as outlined under the Ministry of Children, Community and Social Services "Policy Directive 6.0 for Application Entities - Individuals in Urgent Need of Support".

The Transitional Support Worker (TSW), as part of the GCSN team, is responsible to coordinate supports for individuals and families who are encountering a range of crisis situations, often have complex needs and who may need access to more immediate time-limited services. Staff are required to do community outreach as well as rotating on-call services. Preference will be given to individuals who have experience working with diverse individuals, families and communities. The Transitional Support Worker role is evolving, and the requirements of the position may change over time.

KEY RESPONSIBILITIES:

  • Implement and coordinate crisis/stabilization response and follow up plan with parties involved;

  • Engage in cross sector systems facilitation;

  • Provide intake support as per schedule;

  • Provide after hours on-call;

  • Assess clinical needs as presented by client, family and/or service provider;

  • Arrange and monitor the work of contracted individual support staff to implement plan(s) created by the service team;

  • Maintain and nurture existing partnerships through regular contact;

  • Manage complex service situations in a respectful and professional manner;

  • Maintain competing demands such as client crises, service provider requests, data entry, and recording schedules;

  • Work independently and collaboratively with an interdisciplinary team and a range of community service providers.

The position requires the individual to work some evenings, as well as provide weekend, statutory holiday, and after hours on-call support on a rotational basis.

QUALIFICATIONS/EXPERIENCE:

  • B.S.W. or M.S.W. with minimum 3 years of experience, or equivalent combination of education and experience;

  • Familiarity working with a variety of multi-disciplinary community services (i.e. ACT team(s), case management, etc.) within the adult developmental sector;

  • Experience in working with individuals, who are encountering service challenges related to linguistic, cultural or religious diversity. Preference will be given to individuals who have experience working with diverse individuals and communities;

  • Experience providing services in "non-traditional" and culturally appropriate, community-based ways which are responsive to the needs of individuals and their families.

KNOWLEDGE/SKILLS:

  • Ability to perform tasks involving physical activity, including lifting, bending and prolonged standing as well as the ability to be trained in CPI (Crisis Prevention Institute) techniques;

  • Organizational, planning and facilitation skills;

  • A self-starter with an ability to work independently in the community;

  • Excellent communication skills, both verbal and written, including report writing;

  • Flexibility, resourcefulness and possess good time management skills;

  • Proficiency in working with Microsoft Office Suite.

ADDITIONAL REQUIREMENTS:

  • Satisfactory clearance under the Vulnerable Sector Screening process;

  • Comfortable working in different settings;

  • Possession of a valid 'G' class Ontario driver's license is preferred and fluency in French or a second language are advantageous.
