We are currently looking to hire a:
Bereavement Psychotherapist
1 – Full Time, Contract until March hours per week)
Reports to: Community Health Teams Manager
Purpose:
Seeking a Bereavement Psychotherapist for our Hospice Palliative Care program to provide direct counselling, case management, service coordination, case planning, and goal setting to clients using evidenced informed practice. Specializing in Hospice related services, the social worker will utilize their knowledge of hospice palliative care, grief and bereavement, as well as health and social service systems to deliver service to clients and families while working effectively in interdisciplinary teams. The Hospice Palliative Care program that hosts a multi-disciplinary team of registered and allied health professionals offering services to people living with and dying from life-limiting illness, their caregivers and family.
Key areas of responsibility:
- Provide comprehensive biopsychosocial assessment, develop collaborative goals and create effective care plans with clients with life limiting illness and their caregivers/families.
- Provide clinically appropriate individual and family counselling, advocacy and case management supports.
- Complete risk assessments and crisis interventions within a psychosocial framework
- Assess grief support needs from an evidence-based model and tailor interventions (social, educational, clinical) appropriately.
- Develop professional relationships at the front-line level and work collaboratively with program partners and service providers on different levels of the healthcare system to ensure effective referral and necessary provision of services
- Maintain an understanding of relevant legislation such as Health Care Consent Ac, Medical PHIPA, etc and their application to practice.
- Understand and apply current norms of practice and best practices in Hospice Palliative Care and Grief and Bereavement.
- Group facilitation and group programming
- Experience working with diverse Communities and complex clients
- Perform other duties as assigned.
Educational and/or Professional Qualifications:
- Regulated professionals acting within their scope of practice (i.e. nurses, social workers, psychologists and occupational therapists.
- Registration and a member in good standing in their respective College.
- Registered Psychotherapist, licensing through Registered Psychotherapist College CRPO, clinical counselling.
- Experience in counseling or providing therapy with individuals and families from diverse communities.
- Knowledge and/or expertise in specific clinical therapeutic interventions such as grief therapy, narrative and dignity therapy and/or CBT; experience providing Complicated Grief Therapy (CGT) preferred.
- Therapeutic skill to support individuals and families to address a broad spectrum of bereavement needs including: loss of parent, partner, family member, children's grief.
- Strong group facilitation skills.
- Excellent organization, communication and interpersonal skills and the ability to work collaboratively as a team member in a dynamic, interprofessional environment.
- Demonstrated ability to advocate for services to meet complex needs of specific populations.
- Demonstrated ability to provide Person and Family Centered Care.
- Maintain accurate and concise confidential EMR documentation pertaining to client history and encounter data.
- Demonstrated initiative and ability to work independently with minimal supervision.
- Current certification in CPR or willingness to become certified.
Essential Competencies (Skills and Attributes):
- Oral and/or written fluency in other languages would be considered an asset.
- Proficiency in the use of computers and various software applications including EHR applications.
- Demonstrated knowledge of health care system as it relates to palliative care delivery.
- Demonstrated problem solving and critical thinking.
- Demonstrated time management and ability to work autonomously.
- Excellence in communication, partnership building and inter professional care commitment.
- Demonstrated commitment to continuous learning and professional development.
Working Conditions:
- Ability to work flexible hours including evenings.
- Ability to travel across Scarborough to a variety of care settings.
- Valid driver's license.
- Car required.
Remuneration: $ $40.99
Band: D
