The Care Company is seeking a skilled and dedicated Occupational Therapist (OT) with strong home and community care experience to join our growing team. This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require in-person home visits with vulnerable community members.

Key Responsibilities:

  • Complete initial in-person home assessments within 24 hours of referral, including functional assessments, mobility evaluations, equipment needs, and ADL/IADL analysis
  • Provide follow-up visits (30-minute or 15-minute sessions) focusing on goal progression, safety, mobility, environmental modifications, and skill building
  • Deliver client-centered care aligned with HCCSS/ expectations, including timely communication and adherence to care pathways
  • Develop and implement individualized treatment plans based on client goals, safety considerations, and clinical best practices
  • Educate and coach clients, families, and caregivers on functional strategies, equipment use, home safety, and risk mitigation
  • Coordinate with interdisciplinary providers (PTs, PSWs, nurses, case managers) to support seamless care transitions
  • Document all assessments, treatment plans, progress, and communications in a clear, timely, and clinically compliant manner
  • Recommend, trial, and prescribe assistive devices and home modifications, collaborating with vendors or contractors when needed
  • Order supplies through vendors
  • Monitor client progress and modify care plans as appropriate to support meaningful, measurable outcomes
  • Maintain communication with the office regarding caseload availability, scheduling updates, and urgent visit readiness
  • Uphold The Care Company's commitment to safe, high-quality, ethical, and compassionate community care
Requirements

Scheduling:

  • Flexible full-time schedule, mainly Monday-Friday, with occasional Saturday/Sunday availability required, based on client need
  • Must be able to complete an initial in-person home assessment within 24 hours of receiving a visit request
  • Clients are located within Central Region (may include Richmond Hill, Markham, Newmarket, Simcoe, Keswick)
  • Ability to manage scheduling independently and communicate promptly with the office regarding availability
  • Travel between client homes required; access to a reliable vehicle is an asset.

Qualification and Experience Requirements:

*Home and community care experience is a MUST. Applicants without community experience will not be considered.*

  • Minimum 2 years of experience providing home and community care, specifically with HCCSS and/or (Ontario Health at Home) clients
  • Registered and in good standing with the College of Occupational Therapists of Ontario (COTO)
  • Applicants who are ADP authorizers will be prioritized.
  • Demonstrated skill in providing functional assessments, equipment recommendations, Equipment and supply ordering through vendors, safety evaluations, and ADL retraining in the home environment
  • Strong understanding of community discharge planning, fall prevention, mobility supports, and caregiver training
  • Skilled at electronic documentation
  • Experience coordinating with interdisciplinary teams and communicating with case managers
  • Ability to work independently in the community setting and produce clear, timely clinical documentation
  • Valid driver's license and access to reliable transportation (or ability to travel efficiently within Central Toronto)
