The ARC program serves persons who are homeless, underhoused or socially isolated, which will include diverse communities served by Sherbourne including, newcomers, 2SLGTBQ, and those with mental health issues, and/or substance use issues.

**about the role**:
Working with the interdisciplinary team, the System Navigator provides resources, advocacy and support, including brief support and crisis intervention to clients during their stay in ARC. They complete a comprehensive social assessment of clients upon admission and work with the client to prioritize their needs during their stay. They coordinate care and facilitate linking clients to appropriate health and social services both within SH and the community.
**scope of the mandate**:
The System Navigator completes social assessments of clients upon admission and re-evaluates their social status throughout their admission. They work with the client and ARC team members to develop a care plan and support clients in achieving their identified goals.
The System Navigator coordinates referrals to health and social supports and facilitates care planning while collaborating with a range of providers to support best outcomes for clients. Referrals may support access to counselling services, income assistance, housing, legal support, food, education, immigration support, employment training, disability assistance, clinical case management, mental health and addictions services, Home and Community Care etc.
The position requires sound judgement in working with high-risk situations (e.g. substance use, serious medical conditions) while operating within trauma informed care, anti-oppression and harm reduction frameworks.

**Systems Navigation**
- Maintains up to date knowledge of services and agencies available to clients, including Sherbourne’s own programs and services, relevant government acts and legislation, and local, national and international trans health related services and resources
- Establishes clients’ eligibility for specific services (e.g., housing, ODSP, etc.) and where necessary advocates on the service user’s behalf
- Engages with clients and existing supports to understand what the client deems as priority
- Follows up with clients regularly throughout their stay
- Assists clients in accessing equitable health care and essential social services and community resources, which may include responding to inbound phone inquiries or accompanying clients to specialist appointments
- Identifies gaps in services or problems in service delivery and works with team members, other organizations and client groups to promote new or improved services.
