Job Posting: System Navigator – Full Time Contract – 1 year with possibility to renew
Organization: Lakehead Nurse Practitioner-Led Clinic (LNPLC)
Location: Thunder Bay, Ontario
Salary: $76,150 annually. We offer extended health care benefits, HOOPP pension (optional), 4 weeks paid vacation in the first year as well as learning and developmental opportunities.
Hours of work: Monday to Friday 8:30 am – 4:30 pm.
Position Summary:
LNPLC seeks a dedicated System Navigator to help individuals and families understand, access, and coordinate the various community, health, and social support systems available to them. This position (supported by the Noojmawing Sookatagaing Ontario Health Team) will be based out of a newly created Thunder Bay Attachment Centre and will focus on guiding individuals through complex service systems; such as housing, income assistance, mental health supports, disability services, or healthcare, to ensures they receive the resources they need (taking into consideration culturally safe care, language, and individuals' preferences) supported by this role. The System Navigator will also liaise with Health Care Connect, and partnering primary care providers to guide them to appropriate interim resources until they can be connected to a primary care provider.

Key Responsibilities:
· Conduct needs assessments to understand the individuals and their family situations.
· Help individuals set goals and create action plans.
· Provide emotional support and advocacy
· Explain how different programs work (e.g. social assistance, housing services, community programs).
· Help individuals determine eligibility and complete applications.
· Connect individuals to specialized services and supports.
· Communicate with external agencies on behalf of the client.
· Coordinate appointments, referrals, and follow-ups.
· Track client progress and adjust plans as needed.
· Advocate for individuals experiencing barriers (e.g. discrimination, inaccessible services).
· Help individuals understand their rights.
- Teach individuals how to navigate systems independently.
- Maintain up-to-date knowledge of community resources.
- Provide workshops or information sessions when required.
Education, Qualifications, and Skills:
- Minimum 3 to 5 years of experience in community health.
· Current registration with the Ontario College of Social Workers an asset.
- Experience with social determinants of health.
- Awareness of and commitment to community health.
- Ability to work with diverse communities with respect and cultural understanding.
- Experience in building and maintaining relationships with community partners.
- Excellent interpersonal, communication.
- Strong time management and prioritization skills.
- Proficiency in computer and word processing skills.
- Ability to work independently and as part of a multidisciplinary team.
- Current BCLS.
Additional Job Requirements:
· Satisfactory criminal records check, including vulnerable sector screening.
· Valid Class "G" Driver's License and access to a personal vehicle.
· Up to date on vaccinations (including MMR and Covid) – must provide up to date immunizations & Covid vaccination record
