We are seeking a compassionate and dedicated Home Care Nurse to provide leadership to a team of HCA's. High-quality patient care in the comfort of our clients' homes. The ideal candidate will have a strong background in nursing, scheduling, Client assessment, problem-solving, and intake assessment. This role requires excellent communication skills, problem-solving, and a commitment to enhancing the quality of life for patients.

Responsibilities

  • Conduct comprehensive assessments of patients' health status and develop individualized care plans.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor patients' vital signs and overall health, reporting any changes to the healthcare team.
  • Adjusting and planning schedules as needed
  • Provide education and support to patients and their families regarding health management and wellness.
  • Assist with and oversee the delivery of daily living activities, including personal hygiene, mobility, and nutrition.
  • Maintain accurate medical records and documentation in compliance with healthcare regulations.
  • Collaborate with other healthcare professionals to ensure coordinated care for patients.

Skills

  • Strong knowledge of patient care practices and medical terminology.
  • Excellent interpersonal skills with the ability to build rapport with patients and families.
  • Proficient in nursing procedures and techniques, demonstrating a high level of clinical competency.
  • Ability to work independently while managing time effectively to meet patient needs.
  • Compassionate attitude towards patient care, ensuring dignity and respect at all times.
  • Strong problem-solving skills, with the ability to adapt care plans as necessary based on patient conditions.

Job Types: Part-time, Permanent

Pay: $26.00-$37.00 per hour
