JOB TITLE: Home and Community Care Coordinator

LOCATION: IRTC Sub-Office, Headingley, MB

TERM: ASAP – March 31, 2025 (with possibility of renewal)

Reporting to the Tribal Nursing Officer, the Tribal Home and Community Care Program (HCCP) Coordinator's will actively work with IRTC's First Nations in the planning, development and implementation of the First Nation and Inuit Home Community Care Program (HCCP). Duties include, but not limited to:

· Participates in the analysis and compilation of data to identify trends, health problems and social and economic conditions that adversely affects the health of First Nations populations that will contribute towards policy formulation, OCAP principles, program development and funding opportunities.

· Provide leadership and direction to planning, organizing, implementing, maintaining and evaluating a culturally relevant HCCP Program. Conduct this in a manner that is respectful of diversity, and in alignment with the culture and traditions of the communities served

· Assist communities within the IRTC affiliated and independent First Nations in developing and implementing HCCP's activities to complement existing services and established priorities.

· Provide nursing supports and assistance to IRTC's First Nation's HCC Workers via telephone/email/in-person in the development and implementation of the client's care plan, assessments, clinical resources, case management, file record keeping, e-SDRT database technical issues, wellness challenges, access to resources/materials/information/training, clarification on FN Health funding, program delivery and other considerations, connection to available community support services and essential health services.

· Identify and develop appropriate resources and services, planning, HCCP strategies and coordinating community programs/events based on community needs.

· Support and provide current HCC information, resources, materials, conferences and workshops to build capacity and community awareness.

· A BN (Bachelor of Nursing), or BSN (Bachelor of Science – Nursing) University degree / RN – Registered Nurse, with 3-5 years' work and/or related experience.

· Proven ability to maintain a high level of confidentiality.

· Must be registered and in good standing with the College of Registered Nurses of Manitoba.

· FNIHB Immunization Certification.

· 3-5 years of experience in HCC services, community wellness, nursing services, case management, working with at risk populations, and preferably within a First Nation environment.

· Must have a class 5 drivers license, reliable vehicle and ability to travel.

· Must be able to provide a Criminal Record Check.

· Ability to speak and understand Saulteaux/Ojibway an asset.
