Position Title: Personal Support Worker

Department: Supportive Housing Department

Reports To: Supportive Housing Manager/Lead

Location: St Matthew's Bracondale House – 707 St Clair West, Toronto

Position Summary

Reporting to the Supportive Housing Manager, the Personal Support Worker (PSW) plays a crucial role in providing personalized care and homemaking assistance to seniors residing in our community. The PSW will assist with activities of daily living, needs and personal preferences, including personal care, light housekeeping, meal preparation, and companionship, ensuring the physical and emotional well-being and comfort of our residents. The position requires compassion, attention to detail, and a commitment to promoting independence and dignity among our senior residents.

Duties & Responsibilities Personal Care

  • Assist residents with activities of daily living as per pre-established Plan of Care, such as (but not limited to daily personal hygiene, including (but not limited to) bathing, grooming (hair, skin and mouth care), toileting and dressing.
  • Assist with stocking of supplies and resident care inventories
  • Assist residents with mobility transitions in and out of bed, from sitting to standing, positioning, etc, ensuring the health and safety of residents.
  • Provide basic medical care such as changing uncomplicated dressings, wound care, and assisting with toileting needs while maintaining the resident's dignity and privacy.
  • Administer medication reminders as per training and under the supervision of the Supportive Housing Manager.
  • Offer compassionate palliative care when required.
  • Monitor and report any changes in residents' health or behavior to the Supportive Housing Manager and other PSWs caring for the same resident.
  • Perform any other necessary personal care services as directed.

Homemaking Services

  • Perform light housekeeping duties such as vacuuming, mopping, laundry and cleaning kitchen and bathroom areas.
  • Prepare and deliver (as needed) individual meals based on resident preferences and dietary requirements.

Companionship and Emotional Support

Provide companionship and engage residents in meaningful activities and conversations to enhance their emotional well-being.

Encourage and support residents in participating in community events and programs.

Work with volunteers, community support providers and family members to ensure quality interactions and continuum of care

Monitoring and Reporting

Conduct & participate in care planning and monthly/quarterly review meetings as required; report any changes in a resident's mental, physical & emotional status and ensure adherence to prescribed medication schedules & necessary wellness checks.

Monitor residents' nutritional needs, conduct safety & mobility assessments, and promote independence through daily activities.

Maintain accurate and up-to-date records of care provided daily and any changes in residents' conditions; document care provided and respond promptly to incidents and emergencies, informing the Supportive Housing Manager and creating an incident report to share factual information & actions taken.

Team Member

Abide by the policies and procedures of Bracondale.

Abide by the Occupational Health and Safety Act, and work safely, reporting incidents immediately to the Supportive Housing Manager.

Address conflicts involving residents or staff, providing timely updates and seeking guidance from the Supportive Housing Manager when needed.

Perform other duties as assigned to meet the overall goals and objectives of Bracondale.

Operate within a healthy workplace culture and display core values aligned to the organization.

Maintain knowledge, training and expertise in relevant fields.

Qualifications and Requirements

Completion of Personal Support Worker (PSW) certificate from an accredited program/community college.

Current/Up-to-date First Aid and CPR certification.

Current Criminal Record Check to ensure the safety, security and well-being of residents/staff

Previous experience in providing personal care and homemaking services, preferably in a supportive housing or senior care setting.

Knowledge of safe patient handling techniques and infection control practices.

Knowledge and understanding of the needs and challenges faced by aging seniors.

Ability to perform light housekeeping duties and meal preparation according to dietary guidelines.

Strong interpersonal and communication skills, with a compassionate and client-centred approach.

Demonstrated empathy, patience, and compassion in delivering care and support to seniors.

Culturally sensitive

Ability to work independently and as part of a team, creating & adhering to care plans and reporting any concerns promptly.

Ability to assist residents with physical transitions in and out of bed, from sitting to standing, etc.

Working Conditions

Interacts with residents, family members, staff, volunteers & community agencies

Intermittent physical activity, including walking, standing, sitting, lifting, and supporting residents

Ability to move/lift 40lbs repeatedly and without restrictions

Ability to sit or stand for long periods of time

Ability to walk, reach with hands and arms, twist, stoop, kneel or crouch using safe body mechanics

Ability to work safely with bodily fluids and potentially infectious diseases, using personal protective equipment/tools when required

Ability to work safely with hazardous materials such as oxygen, cleansers, medications, etc.

Understand that working conditions may result in contact with smoke, hoarding, verbal inappropriateness, unpleasant smells and living conditions, illness, dying & death, etc.

Directly reports to Supportive Housing Manager/Lead, you will also need to maintain and complete data collection daily/weekly/monthly & quarterly

Participate in reporting as needed to support the team to fulfill the clients' service goals.

The Personal Support Worker will work with a variety of partners and client/caregivers and maintain timely and transparent communication with them to create a clear understanding of all timelines, activities, and outcomes.

Job Type: Part-time
