Behaviour Support for Seniors Program in the TC-LHIN is based at Baycrest Health Sciences and is mandated to build capacity across sectors to support residents with responsive behaviours. The program is seeking to hire a Behaviour Support Lead to work in Apotex, which is a Baycrest Long Term Care Home with neighbourhoods that include a Transitional Behavioural Support Unit (TBSU), Physical Support and Mental Health neighbourhoods. Our staff members support residents with a wide range of conditions common with ageing such as physical (e.g. from ambulating independently to being wheel-chair bound), mental health, cognitive (from mild to severe including Dementia and Alzheimer's).

TBSU is a 27-bed unit with 23 beds that are specialized transitional behavioral support beds as stipulated by the Fixing Long-Term Care Act, 2021. The residents in this neighbourhood receive a higher level of support than they would in other long-term home environments. The staff focus is on improving quality of life and outcomes for residents with exceptionally challenging behaviors while working in a collaborative, innovative, and supportive atmosphere.

The Behaviour Support Lead (BS-Lead) will serve as an in-house resource on identification and management of responsive behaviors. The dedicated BS-Lead will assist with the management of residents presenting with or at risk for responsive behaviours that may be associated with complex and challenging mental health, dementia, complex mental health, substance abuse and/or other neurological conditions.

The BS-Lead will work in collaboration with the, Psychogeriatric Resource Consultants from the Geriatric Mental Health Outreach Team, in-house Advance Practice Lead for Responsive Behaviours, Long Term Care Behaviour Support Outreach Team, hospitals, Home and Community Care, primary care providers, and other providers and professionals.

Responsibilities include but are not limited to:

  • Enhances services for individuals with behaviours associated with complex and challenging mental health, dementia and/or other neurological conditions
  • Participates in the recruitment of staff, collaborate with the managers on completion of employee performance appraisals, and support the day to day operations of the home
  • Collaborates in managing and co-ordinating Responsive Behaviour program at The Jewish Home for the Aged - Apotex
  • Provides training, supervision and support to the Behaviour Support Team, and will assist them in planning, implementing and evaluating behavioural interventions for individuals who present behavioural challenges
  • Maintains accurate and complete records of clinical observations and care. Complete and submit BSO Lead tracker on quarterly basis
  • Works in compliance with College standards, legislative requirements and the policies of the Home
  • Participating in Mandatory BSO Lead training and regional Leads' meetings
  • The BSO Lead will be expected to demonstrate flexibility in undertaking additional duties as assigned.

Qualifications include but are not limited to:

  • Registered Practical Nurse with Current Certificate / Degree in good standing with College of Nurses of Ontario required
  • Certifications in Dementia Care, P.I.E.C.E.S., GPA, U-First required
  • Current Basic Cardiac Life Support ('BCLS'), Level C
  • Recognized First Aid certificate
  • Certificate of Medication Administration or equivalent
  • Minimum one (1) year experience with residents in a long-term care setting
  • Supervisory experience in Long Term Care and/or Mental Health environment
  • Demonstrated familiarity with the Fixing Long-Term Care Act, 2021 and Regulations, Mental Health Act, Occupational Health & Safety Act, Restraints and Code of Conduct
  • Demonstrated ability to coach, mentor, supervise and provide performance feedback
  • Ability to implement best practice in dementia, delirium, mental health issues and their effect on persons, families and caregivers, based on Scope of Practice
  • Demonstrated skill in contributing to assessment of dementia, cognitive impairment and delirium, mental health, addictions and physical assessment skills and the techniques of behavioural approach and evaluation
  • Sound understanding of and commitment to the principles of the Behavioural Supports Ontario (BSO) project
  • Familiarity with CIHI's quality indicators an asset
  • Strong knowledge of InterRAI LTCF
  • Well-developed written and oral communication skills with highly efficient computer skills
  • Proven organizational, leadership, interpersonal, & communication skills
  • Ability to take initiative and make independent decisions
  • Able to react to emergency situations appropriately

Compensation:

Our salary ranges are structured to support progression, from learning the role to demonstrating full proficiency.

For this position, the salary range is $58,386 to 72,983. The target hiring range is $58,386 to 62,063 per year.

Your placement within the target hiring range will depend on your job-related knowledge, skills, abilities, and relevant education and experience, as well as considerations such as market conditions and internal equity. The starting salary will be determined using these factors to ensure fairness and consistency across the organization.
