Job Summary:
Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Central regions' Behaviour Support Action Plan. The Clinician leader will work in the coordinating office to lead system navigation, capacity building, data collection, and intake and triage of referrals across sectors.
Responsibilities include but are not limited to:
- Provides clinical operational leadership support to Toronto Region Behaviour Supports Ontario (BSO), including supporting clinical triage and navigation for the central intake of referrals to Virtual Behavioural Medicine (VBM) and BSO programs across all sectors, focusing on optimizing care delivery on specific cases and on the BSO active patient population as a whole
- Collaborates with the VBM care team, client, care partners, and the client's circle of care in the provision of VBM services to optimize care co-ordination and provides advice, recommendations and solutions on the most complex cases
- Maintains and monitors BSO data collection and reporting on a number of accountability measures.
- Escalates issues to the Clinical Manager as appropriate
- Acts as a clinician leader working between BSO coordinating office, BSOT and VBM clinical team
- Leads the BSO coordinating office Team Case Review meetings
- Monitors and reports weekly on BSO Wait times, number of referrals, and other data requested by the Clinical Manager
- Identifies opportunities and takes leadership role to establish linkages with partnering organizations to promote and advance the BSO program and model
- Coordinates and ensures the effective use of resources to meet client needs.
- Participates in relevant education and research activities and performs duties consistent with the job classification, as required
- Reviews complex referrals in collaboration with BSO coordinating office, Toronto Region
- Work side by side with Toronto Ontario Health atHome (OHaH) teams and other community providers to provide behavioural support expertise, coaching and building capacity through partnerships
- Work with the Baycrest Behaviour Support Resource Team to help with system navigation support for clients with responsive behaviours along the care continuum to make referrals to other appropriate supports as required (e.g. Community Psychogeriatric Teams)
- Collaborate and liaise with hospital, community groups, crisis teams, and other community teams to develop an integrated pathway for clients
- Facilitate transitional support from community into acute care or the long-term care sector
- Perform teaching responsibilities, participate in relevant education and research activities, and perform duties consistent with the job classification, as required
- Perform departmental responsibilities to contribute to the efficient and effective provision of service
- Ensure the protection of client rights and acting in an advocacy role for the resident/clients
Qualifications include but are not limited to:
- Completion of a Master's degree program in a healthcare profession
- Current registration/licensure in good standing as required by a professional body
- Minimum three (3) years related experience in geriatrics, community-based seniors care and behavioral management
- Experience/familiarity with Resident Assessment Instrument—Home Care (RAI-HC) and behavioral, cognitive, psychosocial, and environmental assessments relating to responsive behaviors
- Demonstrated expertise and knowledge of working with individuals with various types of dementia, delirium, and those with acute behavioral change
- Demonstrated competence in the use of standard psychogeriatric assessment tools and behavioral support management approaches
- Demonstrated excellent clinical-reasoning, critical thinking and problem-solving skills
- Training in or knowledge of Gentle Persuasive Approach (GPA), P.I.E.C.E.S. U-First, Montessori Approach
- Effective assessment, intervention, client centered and collaborative care planning skills
- Experience providing caregiver education, support and counselling relating to caring for a person with responsive behaviors
- Demonstrated experience working collaboratively with primary care and specialists in the care of older adults with responsive behaviors
- Ability and willingness to travel in the Toronto LHIN area
- Proficient computer skills (Microsoft Office Suite) and the use electronic communication devices and software pertaining to risk management and RAI documentation
- Experience with program development, evaluation and quality improvement initiatives
