About the role:

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

The Nurse Practitioner (NP) works within the framework of nursing as member of the inter professional healthcare

team who provides care related to the prevention of stroke, post acute care of patients who have experienced a

stroke, a suspected stroke, Transient Ischemic Attack (TIA) or Mild Non Disabling Stroke (MNDS). The NP will work

cohesively and in collaboration with the Stroke Prevention Neurologists, Dietician and the regional community stroke

prevention team. The NP role is focused primarily on clinical practice requiring triage, advanced clinical assessment,

clinical reasoning, pharmacology, and communication skills to formulate optimal stroke prevention, post stroke care

and wellness of the patient. The NP will also provide leadership and support to the regional stroke prevention

community nursing staff and will partner with regional stroke strategy centre staff related to projects.

The Nurse Practitioner reports primarily to the Manager of Stroke and Neurology Services, and has a matrix

relationship to the Director of Nursing Practice. The NP will have a professional accountability for quality and safety

to the Medical Advisory Committee through the Chief of Departments and matrix relationship to the Chief Nursing

Executive. The NP is an expert practitioner who functions with a high level of autonomy, with advanced knowledge

and decision making skills to formulate clinical decisions to manage the health care of patients with or at risk of

future stroke through an interdisciplinary team approach. Care outside the NP legislated scope of practice will be

authorized through the use of medical directives and direct orders. Neurologist(s) will partner with the NP routinely

to provide direction related to development, implementation and amendment of patient care plans. NP practice

addresses five domains of Advanced Practice Nursing: Clinical Practice, Collaboration, Research, Leadership, and

Change Agent. The NP in this role leads and chairs the stroke prevention regional committee and mentors clinical

teams providing stroke care in the Northwest region. The NP participates in research as well as provincial and

national committees related to stroke care.

Employment Requirements:

* Current unencumbered registration with the College of Nurses of Ontario in the Extended Class (EC)

* Membership in NPAO or RNAO required

* Successful completion of Graduate Studies

* 3 years recent nurse practitioner experience in an specialty care setting during past 5 year period

* Clinical expertise in anticoagulation, antiplatelet, lipid, blood pressure therapies and lifestyle management

* Experience leading clinicians relating to best practices

* Nursing experience in stroke, chronic disease management, or cardiology would be an asset

* Neuroscience and stroke best practice knowledge would be an asset

* Experience chairing meetings and/or committees would be an asset

* Experience participating in and/or authoring research projects would be an asset

* Demonstrated commitment to the principles of the Patient and Family Centred Care model

* Knowledge of the research process and ability to apply research to clinical practice

* Strong leadership ability and highly developed interpersonal skills

* Ability to manage and work collaboratively within an inter professional team

* Ability to be flexible and adapt positively to constantly changing situations

* Excellent communication, negotiation and conflict resolution skills

* Strong organizational, critical thinking and problem solving skills

* High level of initiative and self direction

* Knowledge of local and regional health system

* Ability to plan, manage and implement change effectively

* Demonstrated commitment to continuing professional education

* Proficient in computer applications and software such as Group Wise, Microsoft Office, Powerpoint and Excel

* Active Professional Liability coverage

* Current Basic Life Support

Salary Range: $67.170 to 74.460
