Team Lead, Secondary Stroke Prevention Clinic

WHAT YOU WILL BE DOING:
The Team Lead is responsible for establishing therapeutic relationships with patients and their families through application of specialized knowledge, skill and judgement, both in assessing health needs and in planning, implementing and evaluating patient care.
You will be responsible for the navigation of comprehensive care for individuals with a high risk of stroke, who have had a Transient Ischemic Attack (TIA) and/or stroke. You will provide advanced clinical assessments, triage, and evidence-based interventions for stroke/TIA patients.
In addition, you will collaborate with leadership on stroke initiatives and hyperacute interventions to ensure safe and effective transitions across emergency, inpatient, outpatient, and community settings. You will be directly involved in delivering patient and family education on risk reduction, recovery, and secondary prevention. You will contribute to program planning, quality improvement, and regional stroke initiatives.

WHAT YOU MUST HAVE:
Five (5) years recent clinical experience in nursing and demonstrated clinical expertise in stroke and cardiovascular pathophysiology and vascular risk factor monitoring and prevention strategies required
- Clinical training and competency for completing appropriate neurological assessments (e.g., large vessel occlusion screening, National Institutes of Health Stroke Scale) required
- Demonstrated clinical expertise and experience triaging patients' risk for stroke / recurrent stroke as per Canadian Stroke Best Practice Recommendations (which includes labs, tests, diagnostics, prevention medication and treatment) required
- Demonstrated knowledge of the Canadian Best Practice Recommendations across the continuum of stroke care required
- Knowledge of community resources to consult for on-going support of patient care required
- Knowledge and expertise of adult learning principles and providing self-management support (e.g., teach-back method, coaching, motivational interviewing) required
- Ability to work independently as well as collaboratively with physicians and other health care providers required
- Experience with electronic nursing documentation within CERNER and ACCURO
- Demonstrated excellent interpersonal and communication skills (written and oral) including knowledge of the legal requirements associated with documentation/health records
- Excellent critical decision making and problem-solving skills
WHAT WE PREFER YOU HAVE:
- Master of Nursing (MsN)
- Canadian Neurological Association (CAN) certification (neuro or rehab)
- Apex Hemispheres
- Stroke Certified Registered Nurse (SCRN)
- Current Basic Cardiac Life Support (BCLS) certification
HOURS:
The successful candidate must be available to work the Secondary Stroke Prevention Clinic hours in accordance with the availability and non-availability requirements outlined in the ONA Collective Agreement. In addition, this position must be flexible to occasional changes in hours when required to meet the operational needs of the department.
